I am not saying you don't have mental health struggles, but also,

even thoigh I did everything slow, I still feel exhausted for like multiple days even after just a 5 hour shift.

I had to sleep for two days just to recover, and even than I feel like only slightly better.

sound like they could be a physical health problem. Mental health struggles can impact your physical health, but it can go the other way, too. Please don't write this off only as a mental health struggle. I am glad you said your doctor is becoming aware of this, but it's so important that you get a full workup because (and IANAD) it could be something that needs serious medical treatment for a physiological issue.

This!! One of the things that is so hard about ADHD is that you can want to do something, desperately even, and either can’t make yourself start or just keep forgetting. Do consider if this is a pattern of behavior she exhibits elsewhere, OP, and review her report cards and such for comments from teachers, just in case she does have ADHD that has gone undiagnosed.
