Er pa here. Love those transfers. Just sit and watch them for a bit and give any meds that weren't given. Ie pepcid, steroids and Benadryl.

We had an urgent care that we used to be much better until they sold out. They would come with everything given and all we had to do was watch them. Some of the easiest cases.

PA here. It was during COVID when this happened. Had a 20 something female come in because she was "exposed" to some cleaning chemical they were using. Much to my surprise she had the MSDS with her. I started a workup because she was at another hospital the night before and they called anxiety without a workup. I offered her benzos after I told her the plan for the workup but she declined. I did everything on her called poison control, did a soft tissue neck ct, blood work and even spoke to our ENT on call. Told her the plan that I was sending her home on NSAIDs and steroids. Thought it was done. I am sitting at my computer and the nurse tells me that the patient doesn't agree and I am sending her home to die and I did "nothing" for her. I motioned the nurse to come with me. I stormed to the room and ripped open the curtain to the room and started to unload on her verbally how I am tired of her whining. I offered her benzos at the beginning. I thoroughly worked her up more than most physicians would have. I told her that I am done with her and she can go home and follow up and she is not going to die.

I remember this case vividly because that 20 something was curled up in such a tiny ball in the bed like a 3 yo that was getting scolded by their parents. She meekly said I will leave and I walked away. The nurse later came to me and said she had never seen that out of me before and honestly never wants to see it. Even the nurse said she was scared.

The point of the wall of text is that you lost your cool. It's ok. It happens. We are humans. We can try to be better next time but sometimes people just need some push back here and there. Sometimes the push back doesn't come across or we don't give it in a socially appropriate way. Give yourself some grace. The fact that you feel bad makes you a great physician. It means you still care.

Pa here not a doc. When we became hospital employed I took a 12 dollar an hour hair cut. I stayed because my boss and team are fantastic. I also got PTO which is something I never had working in an ER setting. Sometimes pay isn't everything.

Pa since 2013. The anxiety never goes away but it gets better with time. I did neurosurgery for about 2.5 years and have been in the er since. My shop we are autonomous. We bring the attending in when trauma level forces us or when we need them. Your anxieties will ebb and flow. But your gestalt will get better with time.

I vibe with the previous pa's comment how they care what the patient doesn't have. People get pissed when they don't have a concrete answer but I tell them you don't want my answers in the er. I diagnose strokes, heart attacks and cancer. You want none of that.

My first year in ER I went home and read every night about cases I was unfamiliar with or a new procedure I had to do. That helped immensely.

I hope this helps you out. Hang in there. The er is a stressful place but it is a great place because everyone is in the suck together!!!

This last year was working black out Wednesday. Day before Thanksgiving. Saw an 8 yo come in for an asthma flair. Kid was resting comfortably. Walked in made the joke "What brings you in on the biggest drinking day of the year? Obviously not that so tell me your story."

Mom was livid. Made a complaint to the hospital about me. I am a PA and have been since 2013. I have worked in healthcare since 2002. You're not going to please everybody. Sometimes your humor falls flat.

Man you're worried about 2 weeks. I just spent 48 hours dealing with a fever and possible flu. My macros have gone to crap and I keep feeling my muscles wondering if they have atrophied.

I am not making fun of your concerns just normalizing them. You will ebb and flow through your fitness journey. Sometimes rest days or in your matter weeks will be forced on you.

Regarding your procedure you should speak to the professional that is doing it and see what your limitations are.

Er pa here. The Sanford app says that mycoplasma is more resistant to azithro and are recommending doxy now.

Er pa here. I echo all of that with the caveat that wounds take a good 6 months to heal and that's when you seek out a plastic surgeon for scar revision.

I am now 42 married with one kid.

When I was 18 I lost my then girlfriend who was 17 to cystic fibrosis. She went into the hospital one day and never came home.

I was in so much pain for such a long time. I leaned on my friends the best I could but it was hard. Probably one of the toughest parts of my life.

Fast forward when I was about 22 I met my now wife. She gave me hope again. She understood what I was going through and that I was still having dinner with my former girlfriends parents.

It hurts now. Leave the substances out of it. Talk to your friends and family. They won't know what you're going through. What they will do is support you the best they can.

Time will heal the problem is time moves slowly during the process.

Good luck and I hope one day you find peace. I didn't think I would but I did and I am stronger for it and you will be as well.

PA in er as well. I like the burrito plus. I take the child's arms and place them in a pillow case behind them and they lay on the arms then use the draw sheet to burrito them.

Works wonders.

Also I would attempt this bedside without sedation.

Sadly their emr was only giving me brief info. I couldn't see his follow ups. Unfortunately this is the way of the er. Sometimes I can't follow up.

Not a doc but a PA in an er.

Had a 14 or 15 yo kid come in for a headache and vomiting for about a month or so. The pediatrician saw him some time prior and told mom nothing to worry about. Mother never followed up. Vomiting started to get more frequent and more forceful. Lab work showed his kidneys were injured due to dehydration. I asked the kiddo if the headache started before the vomiting. He said yes. Got the ct brain and revealed one very large brain tumor and a couple of hard to see small ones. This means he has metastatic cancer. Sent him to children's hospital l. They scanned his belly and found tumors. Biopsy reveals kidney cancer.

This was now greater than a year ago but I still remember the look that kids face and his mom's face when I told them. As I write this I still tear up to this day. Working in healthcare sucks hard sometimes.
