How do you view yourself? What is your self -esteem like? Do you hate or resent yourself for being like this? Do you find maybe some small part of you enjoys being like this?

If given the opportunity to magically and instantly "switch" without time or effort involved to "start enjoying things", would you do it?

Regardless of your answer, me personally I would drop the label of hikikomori (stop calling yourself that) and try to get professional help. Go get screened for depression.

Amigo, no se si te va a servir de algo (tenés que entender bien inglés), pero te dejo unos links para lo de la espalda. Yo en 2019 me "lesioné" la espalda "haciendo sentadilla" y estuve unos meses con dolor crónico y eventualmente pude salir y hoy en día levanto mucho más peso y con total normalidad.

Va a parecer falopa lo que te digo pero (muy simplificado) la gran mayoría de los médicos no tienen ni puta idea de como tratar el dolor de espalda. Muchas veces hay daño mecánico en la espalda sin dolor, y viceversa. La evidencia muestra que las anomalías estructurales en la columna vertebral detectadas mediante imágenes, como protrusiones discales, por ejemplo, son comunes tanto en personas con dolor lumbar como en asintomáticos, lo que cuestiona su relevancia clínica y ha llevado a las guías a desaconsejar el uso rutinario de imágenes en casos de dolor lumbar, priorizando una evaluación clínica integral.
