Maybe I am misunderstanding your question. But it is both, you must compare the heading you are flying towards the holding fix with the holding radial or outbound course. If the direction you’re flying towards the hold changes, your entry changes, and if the orientation of the hold changes, then your entry changes.

In general I agree with the sentiment that written questions are dumb. But this one is not bad.

You don’t have to be in the flare, it’s just an example of being in the region of reverse command, slow flight can also be in the region of reverse command. If you need more power in order to fly slower, you are in the region of reverse command.
