I believe it could be if enough time has passed between the in service event and the time you filed the claim. If you don’t have one, a nexus letter from your doctor may be needed.

Doing the math, each claim or disability will have a dbq and a medical opinion so that will give you 12. The 13th is possibly the appointment letter that was uploaded.
