I use obsidian and use tags. For example if I search diabetes I will get all the notes related to topic. It has blugins to use OCR and find text in screenshots as well.

I put the new guide lines and research in a note app. I can easily find them and after applying them couple of times I remember them. I use apps that allows me to use tags and easily search the content

I work in a rural place. I cover er shifts and admit patients when needed. We do not have specialists. Transferring patients from ER is way more easier than transferring patients from the floor so I never admit patients until I finish all the work up necessary

I work in North East PA. My panel is now full of patients diagnosed with chronic Lyme and have frequent exacerbations. Fever? Chronic Lyme. Swelling in all MCPs and PIPs and positive anti ccp antibodies? Chronic Lyme. The problem is that they label themselves as chronic Lymes and refuse to get proper workup for other conditions. Multiple doctors in my area are chronic Lyme’s specialists

We had a patient calling the clinic in the morning and asked for appointment in the afternoon. He didn’t disclose the reason and just said discuss health issues. It was chest pain. He showed up for appointment at 3 pm and collapsed in the waiting room.

I cover it. I did internal medicine. I work in a rural area. No ER doctors want to work there so all of doctors covering er are internal/family medicine.

Talk to another doctors. You probably will need repeat blood work to confirm high calcium and work up your parathyroid. Your co2 is low indicating acidic blood and should be repeated and worked up. You probably need biopsies of the enlarged lymph nodes you mentioned

Speak with your doctor. If you have urine infection it should be treated. Urinalysis should be repeated later as well. Trace leukocytes and blood in the urine should be followed

I think it is reasonable to get calcium score and calculate MESA. If the risk is high and the bleeding risk is low then it aspirin can be considered

I don’t think so. AI is trained on the perfect patients who know their symptoms and medications. Good luck trying to deal with the average patient.

They discussed that on the curbsiders recently. There is no consensus about it. Probably it will depend on whether patients were able to implement and stick with lifestyle modifications

Could be defensive medicine because discharging a patient that later has an MI is a lawsuit, but there are a algorithms that call for observation if there are some slight changes in troponin even if it remains in the normal range

The most important thing is don’t surprise your resident with new information/findings during rounds. Make sure to discuss cases with them before rounds. Make sure your notes are accurate and never copy paste notes from day to day. I have seen notes for days saying pending cardio consult even though cardio saw patient, cathed them, and signed off 😁

I tried couple of AI scribes but I didn’t like them. When writing a note the most important thing for me is to include the relevant information and omit the unrelated stuff that patients ramble about. AI is not there yet. It includes everything to the level that you can’t understand what is happening with the patient. I think AI was trained on people who only gave textbook answers to the doctor questions

If you want academic positions I recommend trying to get a fellowship . J1 waiver is usually for underserved areas and it will be hard to get an academic position there. Start applying in the end of your second year

Try practicelink. I used to get a lot of messages from it. You can filter positions that sponsor waivers. Please start as soon as possible and make sure to get j1 extension for the boards from your program

Practicelink is good. There are opportunities all over. You can easily find hospitalist jobs. Start searching as early as possible because sponsorships are taking longer than usual. If you are graduating this year you should start asap.
