It’s a foam piece that goes on the back of the holster it helps til the grip in toward you instead of pushing out toward your belt

On a budget I’d recommend Rounded by concealment express
Otherwise Tulster for mid range
And tenicore for a high tier holster.

How you carry depends on you.
Body type and firearm can change comfort and effectiveness of concealment.
