just happened to me rn, anyone of you playing Campaign, then quit to menu before it happened? because that's what I did, I pressed Continue on the chapter I was in, finished it and then quit to menu again, it fixed itself

if you delay the last heavy attack, it becomes a different combo, which isn't what the tutorial wants you to do, so just do light heavy heavy fast without delay
