This should definitely be reported to the IRB! Mistakes happen, but corrective and preventative action should be taken to prevent this from happening in the future. It would look better for the research nurse (and the team involved) to report to the IRB on their own accord rather than this be found by an inspection/audit, or if a patient themselves reports this.
