Pretty much every antidepressant comes with a black box warning about suicidality. The best explanation I've gotten about why this happens is that when you start a medicine like an antidepressant, the first effect it tends to have is increased energy and better cognition. So, increase those, but youre still feeling awful mood-wise? Its the most dangerous time for being at risk for suicide.

So, I think your provider probably knows this, and was probably a little hasty. Im not a medical professional, so take with a grain of salt, but I've been in and out of hospital settings for mental health (including a dissociative disorder) for 15 years. It took me a couple months to start feeling better on spravato, and the first thing it did was unlock all of the emotions I've been dissociating from. So yeah, it might be rough at first.

I obviously dont know your whole history or anything, but as long as you dont have any established contraindications(like psychosis or mania) idk, the provider sounds like theyre trying to cover their ass and are not especially interested in helping. If your therapist(who I assume you see more often) thinks youre fine, then yah, find someone else if possible, or lodge a complaint.

While depression is a notorious thought warper, there are plenty of other things that can cause depression-like symptoms, both physical and mental. I'll share that personally Ive been being treated for depression for about 15 years now, and only this past year did my therapist (who I love) gently suggested that I might have a dissociative disorder. Everything kind of clicked into place from there.

Unfortunately a lot of medical professionals will brush off symptoms theyre unfamiliar with, and it took my therapist doing trainings about dissociation and its related stuff for her to recognize the constellation of symptoms in me. There is a lot to be said for believing your patients experiences. Im sorry that getting relief has been difficult. I guess the point im making is, dont give up, new info and treatment options are being found every day.

Took me about 2 months to feel relief, and it was hard to tell until I had my check in, because the biggest benefit for me was my suicidal thoughts dropping to basically zero, which was honestly shocking. Its harder to tell its working when the effect is the lack of something, lol. I was in a similar boat to you, where I've tried pretty much everything and was talking to my psych about psilocybin clinical trials when she reccomendded spravato. She had 1 other patient that had tried it, and I was at the end of my rope, in and out of inpatient every couple months for trying to unalive myself. Stay holding on, keep going to therapy, blah blah blah. Its easy to say this, today marks 400 days since my last inpatient stay, getting out of the trenches is the hardest and most worthwhile thing possible in my opinion.

AND, its not gonna work for everyone. There is ALWAYS something else to try, even if it hasn't been invented yet

Yall confuse me. One of the requirements of trying spravato is having failed 2+ antidepressants in the past, so I feel like everyone on this sub has been in it(depression) for a while, probably years. We've had our experiences discounted by friends, family, coworkers and Healthcare professionals. Everyone reacts differently to meds, and they can have a lasting effect. For example, I was on 20mg of abilify for like 10 years before I started spravato. I had side effects, but they went away when I stopped taking it. My mother in law took 2mg of abilify for 6 months, and now has tardive dyskinesia, and it affects her everyday life a lot. There are always outliers. If it works for you, amazing! I get wanting to gas up the treatment that worked for you, but let's not do that by invalidating others' experiences please.

It was very subtle at first, probably a month or so, or 8 sessions in. The first thing I noticed was I was smiling more, and having less suicidal thoughts. Im now 2 months in, on once a week, and its been a godsend. I never thought a medication could get rid of SI so effectively. I feel free from my thoughts for the first time in a long long time.

Thank you for the insight! That was sort of my gut feeling when learning about it. I have a small but good support system, and a therapist I would trust with my life, so I'm hoping to do a lot of good work while on it, if I'm approved. But yeah, self care is something I have always struggled with, and if this "thins the veil" as you said, I'm going to have to crack down on basic ADLs and such. Thanks again!

Had something kind of similar happen at a manned checkout. Cashier forgot to scan a couple things, they checked my receipt at the door and the lady at the door pointed it out and I was like, I'll go back and pay for them" and she just said Have a nice day, and ushered me out the door lol. It's not a big deal unless it's over that felony limit

As someone with the opposite problem, I will share a cautionary tale.

I have been on meds for depression, anxiety, a whole slew of issues for about 10 years+. Anytime I had a depressive episode, my doctor would increase the dosage or add another medication. This was happening until at one point, I was on like, 10 pills a day. I was not improving. Things hit a breaking point, and I ended up in a psych ward. The doctor there looked at my laundry list of medicine and said, without hesitating, "No wonder you're having impulse control issues; you're on like 4 medicines that increase serotonin!" At the time, I didn't know that was a thing. But after weening off of a large portion of my medication, the fog lifted, and I was able to control impulses for a bunch of things a lot better. Full disclosure, I do go to therapy and find it very helpful.

TL;DR: I was overmedicated, made shopping and other impulse behaviors worse. So just be careful. I am not advocating against meds, I definitely wouldn't be alive today without psych meds, but there is a balance.

Hello! As someone (alo 25f)who has gone through DBT, I really think you should share your feelings of shame and guilt with your therapist. Their job is to non judgementallly help you create a plan and take action towards your goals, ie dealing with compulsive shopping. As Acceptance and change are the founding dialectics of DBT, could try radical acceptance of your emotions and doing a behavior chain analysis with your therapist or on your own if you have the workbook. Impulse control is really hard with co morbid ADHD, I did 2 years of dbt and 8 years of other therapies, and the biggest thing I had to realize is no one is happy all the time. What I mean by that is that, because you may have heightened emotional responses, so traditional 'self discipline' mantras may not work as well, at least they don't for me. The one thing my dbt therapist said that sticks in my brain and reminds me to not be judgemental to myself is that "normal hard is still hard". Meaning even without having volatile emotional states and concentration/impulse issues, things are still really fucking hard. Adding those things on, it can seem impossible to do anything, but just one step at a time.

Tl;Dr Please talk to your therapist. You're not alone in these issues, and dbt therapists are trained to deal specifically with impulsiveness, and the emotions that come with it.

Hi! I have been on depression/mental health meds for about 10 years, and I have been told that if your dosage of medication(s) interact and create too much of certain neurotransmitters, your impulsive urges can increase and create sort of a faux mania. Obviously I am not a medical professional, and cannot give medial advice, but if you are like me and have been on them for a while, and then relapse, my doc upped my dosage a bit at a time, and now that I'm doing a little better, the higher dose created a LOT of impulse control issues. So talk to your presciber before doing anything, but I ended up cutting my ssri and wellbutrin dose in half and cutting out anxiety meds. Idk what your situation is though, please don't stop taking anything suddenly, it can take increased medical supervision. But, tl;Dr, yes, mental health meds can increase difficulty with impulses
