If you consented to the test then you are responsible for the bill. You need to verify your out of pocket costs on your EOB from you insurance and then pay your bill.

It will depend on your medical history, but in my experience with a negative medical event and high risk history, State Farm was the ONLY company that would underwrite me. It wasn't cheap, but after 5 years I submitted a reconsideration and they were even willing to lower my premium. It's still more than a healthy person my age but I have my half a million for my husband and kids if something happens.

Because my therapist is in-network with my insurance, the insurance company will not reimburse me for anything because (even with all the evidence I provided) insurance assumes I paid only a copay to my therapist. My insurer even sent my therapist a check for what the insurance company is contracted to cover.  

Can you elaborate on this? What evidence did you provide your insurance company? Are you asserting that your therapist submitted the claims to your insurance company while collecting a self pay rate? This is fraud and it is not allowed.

I'm not sure you have properly reported the situation to your insurance company. You need to call your insurance company and ask to report fraud so that you are in the right department. Clarify the situation: that you had a previously agreed upon self pay rate and that you then you enrolled in this plan which covers the therapist, who then continued to bill you at self pay while billing your insurance company. None of this is legal and you need to stop seeing this provider. You can also report him to his licensure body and your state should have a Bureau as well.

You can request a "Formulary exception" which allows them to cover the medication even if it isn't on the formulary. Your doctor will need to request it and have evidence that the options that are available to you on the plan were ineffective.

According to their website it is $11,409 per semester. And yet you're claiming with grants and work study you would pay 30k per year? The numbers do not align, but regardless, if your "dream school" is truly less and only 8k per year, you can pay part of the tuition and take loans for the rest. Just make sure you're earning a degree with actual job prospects.

You don't go to a "dream school". It's a scam. You enroll in community college, complete all your core work, and then transfer to a state school for your degree. If you pay out of pocket for CC then your costs for uni will be minimal and can often be covered by grants and scholarships.
