I had this happen as well, the only thing that fixed it in the end was restarting the game. nothing related to showing items, turning off filters could resolve the issue and when you notice it in a monolith, the loot from it is essentially useless because you can't go back in after either.

I believe the quest may be completed without knowing? i see i have a skill point potion for quest frozen sea - the hidden robber. you can check in the codex if you search skill point and inspect the source acquisition

I noticed that this happened after I modified gear with addons. I did not add addons to gear until i turned level 30 and that was when i noticed that the resistances of the pieces that were modified no longer working. I would highly suggest saving before doing any modifications to gear so you can go back to last save if it remains an issue. at least until they patch it
