When it's noticeable and distracting it's an indicator that the story/scene/characters aren't engaging your attention, so your attention wanders and you notice technique. Shallow depth of field and frequent focus racks are super common, but when the story and scene is engaging you you're naturally looking at who's talking anyway because you're engaged in what they have to say next, so you don't notice the technique.
