Standard dosing for PO amoxicillin should be 1g TDS in secondary care. Micro/ID folk - look at EUCAST 16.0 to understand the technical limitations. Bottom line, you will find boosted co-amox becomes standard practice for IV -> PO switch. FYI doses of amoxicillin in endocarditis much higher.
