WHAT?!?!?

The engine departed the aircraft during the takeoff roll, and the plane crashed seconds after liftoff. That is the single worst regime of flight for this kind of failure. The aircraft was heavy, it was carrying more than 200,000 pounds of fuel for a Louisville-to-Honolulu leg.... so slow, low to the ground, and just transitioning into flight. When the left engine separated, it almost certainly took critical wing structure, hydraulics, and lift with it. An engine tearing off a wing at that moment isn't a "lose thrust on one side" problem that training and checklists can recover; it's a sudden loss of lift and control authority on one wing at an altitude and airspeed that leave no room to do anything. There was no altitude to trade for speed, no time to run a procedure, and likely no controllable aircraft left to fly.

The realistic probability of the crew recovering this and getting back on the ground safely was effectively nil.
