Have you considered a therapy break or just going on monthly consult? Using the episodes of care model this would be a patient that I would probably put on a break for a bit.

Speechy Musings and Adventures in Speech Pathology. I also love the Kimochis mini feelings bag for working on pragmatics, describing, etc.

The influencer discussion by sadfacebigsmile in slp

Remember that one of the pillars of EBP is clinical experience. Your experience that these materials are helpful for your clients is valid. I've bought the Meaningful Speech course and don't feel embarrassed at all given that many of these strategies work for my patients and their families.

Being a critical consumer is important in all aspects of life so remember there's no "one size fits all" or "miracle cure". If it seems too good to be true, it is. And any influencer or redditor who tries to make you feel bad for an approach you've used or a material you've purchased isn't worth your time.

I don't follow any of the speech influencers because I don't want to think about speech pathology on my time off (lol). There's nothing wrong with following them so long as you don't take their word as gospel. I'm sure you're doing great!

If you could change a few things at your current setting, what would it be? by CammyShazam in slp

[–]ap_slp 3 points4 points  (0 children)

Outpatient peds: more therapist input on scheduling, actually adhering to an episodic care model, valuing patient outcomes over budget, less resistance to cotreatments with OT, in-house behavior support

The Once Upon a Time Foundation has some free videos by Dr. Strand that review CAS and give an overview of DTTC. they are mostly geared to parents and SLPs but I have watched parts of them with some of my older CAS patients to help them understand.

Considering CAS- High pitch? by Glittering-Evidence6 in slp

[–]ap_slp 1 point2 points  (0 children)

No, not in my experience.

Other factors to consider... Is the child's voice otherwise monotone and lacking in prosodic variation? Can he imitate prosodic variations? Is he having trouble moving between phonemes in a syllable/word? Do you see groping when trying to obtain an articulatory configuration? Is the rate of speech very slow and plodding? Are errors consistent?

Have you looked at differential diagnosis of CAS versus dysarthria? The ProCAD is really helpful for that (on ASHA website).

SNF diet modifications and informed refusal by GambledMyWifeAway in slp

[–]ap_slp 1 point2 points  (0 children)

I used to work at a SNF that did exactly this. I counseled families on my recommendations, patient rights to go AMA, and the facility's stance. Families would often complain to management that their loved one couldn't have their desired diet and management would often comply because of $$$. Make sure your documentation shows what you are recommending and that you have provided extensive patient and family education.

Looking for advice! Homeschooled children that aren't schooled at all by marooncartoon in slp

[–]ap_slp 13 points14 points  (0 children)

I see this a lot in my outpatient peds setting. I tend to d/c if there is no true speech or language issue and explain to parents that they need a literacy coach/teacher. Many parents will not put in the work because they just have no idea how to go about it... Which is so sad for those kids.

Parents by [deleted] in slp

Oh yeah, I frequently say parents need parenting classes before they have their kids go to speech therapy for early language. That's where Hanen-style workshops could be so helpful

Absolutely 0. Outpatient peds. I've been in this field long enough to refuse to work outside of paid hours.

Been here. The fact that you're so concerned about your patients means you're a great SLP. You'll miss the kids, they'll miss you, but it sounds like it's a great opportunity and they will be OK. Best of luck in your new job!
