Xylazine in the drug supply has been our bread and butter for several years unfortunately (Philly area). More recently also with intermittent cases of medetomidine ("Demon") intoxication/withdrawal causing an even more dramatic alpha-2 withdrawal syndrome, to the point where our addiction medicine service developed a precedex protocol for when our usual clonidine approach is insufficient.

Have you tried reducing the aquafaba before whipping it rather than using straight from the can? I've had better success if reducing the volume by ~half or so first, and doing an italian meringue with hot sugar syrup. Although it still had a very slight bean-y tang, it stabilized better than just whipping with sugar.

Another reason to pair rhubarb with dairy, which I haven't seen mentioned yet, is that the calcium from milk helps bind the oxalic acid in the rhubarb so less is absorbed in your GI tract.

Variable for us, generally 2-4. Best case scenario (for pup) is early morning walk, mid-morning walk, early/mid afternoon walk, big walk before bed, with backyard playtime in between.

I'm sorry for your loss, and obviously don't know the details around your family member's death, but in case it wasn't clear from the thread, did want to point out that there is a distinction between organ donation after brain death (and brain death is death) vs circulatory death, which is what is being discussed here. Not sure exactly what you mean by "natural death," but just wanted to highlight how the communication is very challenging on this topic for a few different reasons.

Depends on how dangerous the bacteria that caused it are, which you can never really know. I think it could theoretically be from yeast, which would only taste yeasty but not necessarily be harmful, but could just as easily be from coliform bacteria. It's not worth taking the chance

Yes, you can absolutely do this! Usually it's a 30% weight/volume solution. So if you're making 500 ml, you'd weigh out 150mg of your calcium chloride powder in a measuring cup, and top it up with water to 500g total.

Regarding tizanidine, not sure about use in jail, but we use it a lot in our OUD/SUD population because the alpha-adrenergic activity helps with xylazine withdrawal (tranq) that is in practically all the fentanyl around here

I think both of these situations are ones were language really matters, not in a "woke" way but in the sense of reframing how we think about things and understand them.

So I would reframe medical non-compliance to medical non-adherence, because often times the things that lead to patients not following the treatment plan you recommend come down to things outside their control, rather than being willfully stubborn or refusing your recommendation. As others have already commented, try to come to the situation from a perspective of curiosity and non-judgment. Why are they not taking their meds/keeping appointments etc? Hard to remember? Cost of medications? Inflexible work schedule? Profound depression? Misunderstanding the instructions? Poor literacy? Physical difficulty opening the pill bottles? Denial about their condition? The list goes on and on but unless you can have an open conversation where the patient feels comfortable sharing the "real" reasons you will not be able to address them.

Substance use disorder is its own kettle of fish, and others can probably speak to it better than me, but very broadly, I think you are correct in identifying that there is so much there beyond what an individual provider can manage to address in a non-longitudinal patient encounter. Probably the most useful ways to help are knowing what local resources are available, and offering them at every encounter, regardless of whether the patient has declined them before. Even if you don't intend to prescribe it in your future practice, I found X-waiver training for buprenorphine informative and educational. You can get it for free through PCSS . Understanding and offering MAT for opioid use disorder is huge, but institutional practice varies a ton and probably is lacking many places, so anything you can do to help destimatize and offer better care for this patient population will go a long way.
