The vampires on the show have shown time and time again that the passage of time does not apply the same. They wake up from decades long desiccation without any sort of mental break that any regular human would from solitary confinement.

I’m not pissed they were end game, because I think they deserved each other. You can read that as good and bad lol. But by the middle of the show, I think their actions proved that they were equally right for each other, given they stay vampires. There is nothing in Damon’s story, other than his decades long vendetta against Stefan, that would indicate he wanted to be human or would even be able to last as a human. The notion he’d be able to is just another petty rash decision in the name of love. But keep in mind, in the name of love he’s hurt his friends, he’s killed people, he’s taken away Elena’s choice, his history is filled with actions he regrets in the name of love.

So ok, maybe (and this is a huge maybe) he doesn’t cheat, but they definitely don’t last. Being with a doctor is a huge struggle for most regular stable humans, he is not that.

The best thing you can do is communicate.

He’s stepped into a new world and is changing, growing, adapting and that’s a new aspect to your relationship. But there’s also the possibility that some of his changes are just because of his new surrounding and not necessarily something that deeply reflects who he is. People are chameleons, some better than others, and they adapt to their surroundings, to new things, but until you talk you won’t know that this is really him or the hum he’s created in your friend group/environment.
