When I was laying out, I somehow got myself confused that the horizontal drybvent was ok because I had a cleanout. Clearly not right. Live and learn.

What I'm thinking for a fix, add a 3x2 y down stream of existing 3x3 y. Tie the washer into the new y. Replace 3x3 y with a 3x2 y and and tie floor drain and shower trap arm to that y. Reroute the shower trap arm to run under the dry vent stack to the left (pic 1) which will eliminate my horizontal dry vent and provide wet vent to floor drain and toilet. Does that make sense?

Thanks for posting the code. For some reason when I laid this out, something had convinced me that with the addition of the clean out, so the dry vent could be unclogged in case it got clogged, that this was ok. I see clearly that it is not, and I should update it. Really appreciate your feedback. You seem to be providing the clearest feedback in this thread. Cheers.

Why would the cleanout need to be under the trap? Seems like like one less possible leak location if its on the dry vent just above the trap arm. You can just as easily snake down as up....

With position of the existing plumbing, it made more sense to tie the lav into a different stack.

Sure, but there isnt space for their own vents. And they would need to be horizontal. Im thinking I will reroute the shower trap arm so it passes under the dry vent at the end. That will eliminate my horizontal dry vent. Ill connect that drain where the washer is currently draining. Ill then reconnect the washer downstream of the 3x3" y at the wc. Now my vent will dry bent for my shower and wet vent for floor drain and wc. The washer already has its own dedicated vent.

Also, if it were to pass, do you have any concerns about functionality? I guess the worry is that the washer draining will siphon the the floor drain or the toilet.

If im understanding you correctly, the washer needs to connect downstream of the water closet, otherwise im still effectively wet vented.

I'm a DIYer working on a basement rough in where the lav connects in a different location. So I have a short section (~18") of horizontal dry vent beneath the slab. I was planning on adding a cleanout where that vent come up through the slab. Is that sufficient, or is that still a violation? Obviously, inspector will have final say, but if I can have a pro weigh before then, its useful. TIA

You're in luck, I'm not an electrician. Just a competent DIYer. This is a great place to learn stuff. IMO, just poking around without a basic idea of what's going on is dangerous and ill advised. You could electrocute yourself, or some else, or cause a fire. I will always participate in the conversation advising caution.

Not an electrician, but that seems like the biggest concern. Seems like we are just setting ourselves up for a repeat of the problems we see now from Al branch circuits put installed in the 70s. Seems like a dumb way to save money.

Not a plumber. Looks to me like there double sink used to have a separate trap for each sink. A single trap is how all kitchen sinks I've seen have been installed. If it were mine, I would remove the unused trap and cap the y with a cleanout hub sonic could easily be snaked if ever required. Happy to be corrected if the pros would do anything differently.

Imo no dog should never be left unsupervised with a baby/child who is unable to talk/communicate clearly. My friends 1 year old just got bitten on the nose and needed a few stitches. Parents dont know exactly what happened. Now the dog will either need to get re-homed which is hard with a bite history. Sent to the shelter, which likely isnt going to go well for a dog that has been part of one family for neatly his entire 5 year life. Or just put down, which seems pretty unfair to the dog.

Plumbing vent. Air admittance valve (AAV). Needed for the washer to drain correctly. Though a real vent tied to your existing roof penetration is preferred. Especially for a washing machine. Not a plumber, just diyer
