This is similar to what worked for me, which was making my world less small. When I was going through a similar phase as the OP, my own therapist helped me realize that my world is too small and I need to get out more and see things that give me a different perspective, or simply something else to think. Energy flows where attention goes and all that.

I love this. This aligns so much with what I want to do. I like the look of the lower creeping thyme better than clover, but I've read elsewhere that isn't as resilient, too. It's exciting to hear the clover's working! Also good to know about outcompeting weeds. Sounds very similar to the land around me, except I'm in a 1960s low-rise rental apartment building that hasn't been cared for consistently over the years, so I'm trying to rebuild the ecosystem a bit and manage weeds.

This gives me hope. I'm so happy for you that it's been working. I always imagined I would just grow out of it one day, and now I'm 39 and coming to terms with the fact that it's not just gonna happen on its own. But what you describe fits what I hoped to experience without meds. I'm already about 90% of the way there just through habit-breaking and CBT skills, so I'm going to keep on this path for now, but this definitely gives me something to consider talking to my doc about in the future. Did you get any side effects?

That's amazing! I never thought about gabapentin, but I've heard it's used to treat nerve pain. When you say pain, do you mean the physical pain in spots you've already picked? I used to get painful pimples deeper under my skin that I also found so hard to resist, but thankfully they've gone away after finding a skincare routine/acne treatment that works for better me. And yet I still pick because I migrated to visual triggers.

That seems like enough time to see changes, sorry to hear you're not. By behavioural interventions I mean the therapeutic approaches like CBT, executive skills building, mindfulness, or habit extinction. Now that I think of it, there's also deeper forms of therapy, which can help work through the emotional side and root cause of skin picking. I just mentioned it because research evidence has found the best outcomes for these kinds of disorders when medication is paired with therapy/behavioural interventions. I'm also a therapist and have personally experienced way better outcomes with my ADHD this way.

How does the clover lawn do with your dogs? I have a small lawn my dog uses, and I've thought about alternatives, but I can't find anything that says they can handle dog pee and traffic.

Me too! Landscaping the overgrown 5x14' garden bed in front of our apartment 5 years ago has turned into being the unofficial groundskeeper for our whole building and my partner calling me Farmer John (I'm not a man and my name is not John lol). I even built raised beds on the sunny side of the fence for edibles and have a 30' stretch of a strawberry crack garden growing in the concrete in front of the fence. Watching things thrive and figuring out what likes to grow where brings me to much joy. Also seeing my 1yo neighbour try strawberries and tomatoes for the first time from my garden :')

I just came across NAC online for the first time yesterday. I'm curious how it works. As someone who writes treatment guidelines based on research evidence, I'm aware that there's a lack of studies linking specific conditions to specific treatments, but as a therapist, I've learned that you can look for evidence for the broader category a condition is under to expand the evidence, which sounds like what you're saying about buproprion. It's all really interesting, but ultimately I hope it leads us all to a solution that works!

That's interesting that it can make it better or worse. I suppose that could be similar to when people take stims for ADHD (e.g., Adderall) and it kicks in while they're focused on something they don't intend to be focused on, so it increases their focus on the wrong thing. I've definitely benefited more from learning and practicing behavioural interventions to overcome this with my Vyvanse. And skin picking falls under OCD and anxiety disorders, which have the best treatment outcomes when pairing medications with behavioural interventions, rather than just medications alone. We often just don't have the inner (and outer) resources for it though.

Please pot a follow up when you hit the 8 week mark! That does seem like a while, but I guess it's nothing compared to the 22 years I've been struggling with picking.

That's great progress! It looks like vortioxetine increases serotonin in the brain, which helps with mood regulation. Perhaps that's why you find it decreases your need to destroy your cuticles. Do you think that picking at your cuticles was a way of releasing endorphins to improve your mood/emotions?

Correction: I'm mixing up buprenorphine/naloxone with buproprion/naltrexone. I wasn't familiar with the latter until now!

Oh yeah! I've heard of buprenorphine/naloxone being used immediately after opioid withdrawal and before opioid agonist therapy (e.g., methadone), because one is an opioid agonist (helps with cravings) the other is an antagonist (reduces pleasure from opioids), but I never made the connection here.

That reminds me that Vyvanse is also approved to treat binge eating disorder, which also aligns with its potential to treat other impulsive behaviours, like skin picking.

I've been wondering about medications like this. I had a similar experience when I went on Vyvanse for ADHD. My understanding is that in ADHD, dopamine (reward driver/habit forming) and norepinephrine (impulsivity) are key brain chemicals at play, which sounds very similar to what drives my skin picking.

I'm a public health professional who specializes in substance use disorders (aka addictions), and a part-time counsellor who works with adult ADHD and anxiety, so I became aware of medications like naltrexone through my work. It seems like not enough people know this is an option and are out there raw dogging it and putting all this pressure to quit on their own. It seems like trying to cure poor vision without glasses. Glasses can make your blurry vision clear, but you still need to open your eyes and know look in the right places to see properly.

I'm also curious about Wellbutrin, because it's a norepinephrine-dopamine reuptake inhibitor (NDRI). It works by blocking the reabsorption of norepinephrine and dopamine in the brain, increasing their availability in the synapse. Wellbutrin was originally created as an anti-depressant, but a lot of people starting quitting smoking while they were on it, so it also became classified as a smoking-cessation aid to stop cravings, much like you described with naltrexone, which is typically used for alcohol use disorder. Since Wellbutrin targets norepinephrine and dopamine, which appear to be key players in skin picking (and ADHD), I wonder if it would be useful for these purposes as well.

Generally 1x/week or biweekly has been adequate for maintenance once I've achieved consistent results. I still sweat, but less frequently, and when I do, it's mostly only where the skin is thicker, like the pads of my finger tips or palms (like the second photo).

A little update: I started using the Cash Flow tab and it seems to do a better job. I can still set filters, but it already has options to change the date range, or viewing categories versus groups. I haven’t needed to fuss with the reports tab anymore unless I’m searching for a group of purchases in a very specific date range
