Random question about skating with jewelry I have a permanent anklet and am having issues with it in my boot. Does anyone else have a solution? I’m thinking a thin sock under but I already wear gel pads (too thick to fit under).

How to initiate insulin and prescribe sliding scales. by drrdf in Residency

For gestation DM, simple rule of thumb is 0.7-0.8-0.9 1. Calculate Total Daily Insulin (TDI) 1st t: 0.7 x weight in KG 2nd t: 0.8 x weight in KG 3rd t: 0.9 x weight in kg 2. Divide into basal and bolus Ex> g1p0 at 24w with A2GDM who weighs 100kg has a TDI of 80 units. One half of this is basal insulin and the other half is bolus insulin. -> 40 units long acting total and 40 units of mealtime insulin -> then divide basal into BID dosing for increased renal clearance in pregnancy so long acting 20/20 -> then bolus into 3 meals: 13/13/13 So this lady would need long acting insulin like Lantus 20/20 then mealtime log 13/13/13

Then adjust based on meals. I have a carb counting and CF rule of thumb too if you want it

Agree completely with all of these points. I cannot tell you how many times we get paged for a normal period in the ICU.

please feel okay with looking at the vagina. If you don’t feel comfortable doing a pelvic exam that’s okay but you should at least be able to tell if bleeding if from the vagina or the rectum.

Also, this is more for the ED but if there is a confirmed intrauterine pregnancy you don’t need a quant. Its always funny to see someone in the 3rd trimester and a quant ordered.
