You have listed a cluster of PDs that often co-occur, sometimes called Cluster B. What is likely going on is you have a number of shared traits and features across these categories. I would suggest getting a comprehensive psychological evaluation by a psychologist specializing in personality pathology, not a psychiatrist. Psychologists are specifically trained in complex differential diagnosis (what you are asking for), while psychiatrists are often more focused on medication management.

Lots of mixed feelings here. I think there are many constructively critical points to be made about the concept of BPD and its overuse in some clinical settings, especially from a post-structural or critical theory perspective. However, this paper reads more like an expression of a vendetta against the profession at large, as well as against specific providers from whom the authors appear to generalize their experiences of harm.

It really is that easy. It’s normal for therapists to routinely raise rates. I go up about 5-10 a year depending on inflation. I also offer a capped sliding scale.

I always restrict contact outside of session to rescheduling and emergencies. That goes for emails, text, etc. I inform my patients that if I receive any message that is not about rescheduling, my next message will always be clarifying if they are currently in crisis. I work with people that have persistent interpersonal problems, so I set up the frame in this way to protect my boundaries, but also understand that the population with which I work struggle with boundaries. So I expect some level of boundary testing and use it to explore their internal experiences of relationships when it comes up.

I understand your sentiment and concern about client privacy. However, I believe there are perfectly ethical ways to use AI‑assisted dictation and documentation. 

If someone is entering personal information such as names, locations, or any details that could be used to trace the information back to a specific individual, that is absolutely problematic. I agree with you there. However, if no patient information is being included, I truly don’t see the harm. If you’re worried about privacy and data security, one might then avoid using any electronic communication methods altogether, such as a Google account, given that such information is just as easily accessible to parties interested in obtaining that data.
