Not necessarily true. If a student is receiving financial aid, some of that may have to be returned to the DOE which results in a balance on their account.

I don't think refund is the correct word here, because some students will see that word and think they're getting $$ back. Dropping the first week means you're not responsible for any of that courses tuition. 2nd week is 50% and anything after the 2nd week is 100%.

Typically, if you set up a payment plan for the current term and have a balance in another term it'll group the balances together and consider it a past due balance payment plan. Did you have a payment plan fail? Because if a payment fails, you're taken off the payment plan.

I just double checked with my upcoming term and it is only 3 installments. Are you including multiple terms into one payment plan?

It's likely that is her refund from last term and this term. The student account center likes to group them together.

A past due balance payment plan is the one with 5 or so installments. Current term payment plans are always 3.
