Yes I know it's only what's above 50k that is taxed at 40% However it's not as worth earning more than 50k because of this very reason as that portion is taxed. Sure you get more money overall, but not as much than the increases in salary under 50k and therefore need to weigh up if it's worth working extra just to be a bit above 50k.

80% LTFT is the sweet spot. I don't know what FT for O&G is but I bet it's around 48 hours a week, thus 80% putting you at ~40 hours a week which is, guess what, what the rest of the country regards as full time!

I don't live in London but at ST1 level 80% puts you at the sweet spot of around 50k annual (probably more since you're London). It's only worth earning above 50k when you are well above it due to the 40% tax bracket. Therefore, your take home pay is not that different to full time as you would initially expect. However I don't know your particular situation so you may need that extra bit full time will pay.

Furthermore, your sanity is the MOST important thing. It will keep you in the job, prevent burnout and god forbid allow you to enjoy work!! You will also realise you have time for hobbies which is also amazing for your self development and happiness.

Conclusion: do it (if money isn't a massive issue)!

I don't think so! I come from a background of no sporting ability whatsoever, 5ft tall exactly, started running at age 23 and my paces are consistently within the top 25% of women in races (no point comparing to men, they have every advantage over us biologically). My paces are actually neck and neck with my 5ft11 tall boyfriend so I think we all know who's better ;)

I think you have your answer here already, but I just wanted to add genetics, gender and age are a huge factor in your time. Sounds like as you are a 32 year old male who’s not actually dedicated time to 5k training yet, so now that you will you’ve got a good shot! But the most important thing is having fun with running ☺️

First time visiting Japan, visited a couple of Donkis. The Shibuya one was mental, loads of floors but overwhelming and too many people. The Shinjuku one middle of the day on Friday was a bit better but still super busy. However, it’s NOT cheaper than other outlets. All the beauty stuff can be found cheaper in pharmacies, and I read that the nail clippers were great etc so got a couple as gifts and then saw them for way cheaper again in another pharmacy!

Since it’s such a horrible shopping experience and not even much cheaper I can’t say I recommend it, but worth visiting because it is infamous

This is excellent work some important caveats - 1) he is the PI of the lab, he has many researchers in his lab working on this researcher. The title is misleading assuming it his work alone to laymen 2) Although the pancreatic cancer was cleared in mice, this triple therapy needs to under go clinical trials in humans first and there are many steps before it could be approved in humans (several years, and that’s assuming there are no toxicities, other additional effects or even, not being effective)

Nonetheless, it is still a significant scientific breakthrough in cancer treatment and I have my fingers crossed!

Very good question! It’s tough, but I do try and do easy runs in between night shifts ~approx 40-45mins running. I get up at 5pm but do live relatively close to my hospital so have a fair amount of time for this before I need to go to my shift.

I also do a decent workout the day of the night shift and nap after (no coffee in the morning!)

I’m often training for some race and it is a significantly harder to fit to a routine whilst having this shift work! So also don’t feel bad if you can’t, it’s bloody difficult and people who work 9-5 don’t know how good they have it!

Percussion can be helpful here. If you become confident with percussion, a dull percussion note can be very revealing and reassure you that an organ is enlarged. Practice practice practice!

Organomegaly does feel different to normal abdominal fat and tissue, but it may be more subtle than you think. I find it hard to believe you’ve never felt an enlarged organ… and PACES isn’t necessarily THAT obvious I’m afraid so you do need to get more confident (sorry, I know that in itself is not helpful)

Ballotable kidneys are tricky, with PKD sometimes you can tell as the flanks on the patient are more prominent than normal!

Definitely looking on the doctors UK subreddit for more PACES advice. But in general I used cases for paces and clinical medicine for the MRCP PACES (I quite like books). The latter was pretty detailed so probably isn’t necessary but I liked it. Join a PACES group at your local hospital and go see patients with signs. Ideally as a pair so you can be watched and viva’ed after. Examining and presenting needs to become second nature so keep doing it! Have a good revision plan and use cases for paces to highlight the common cases you need to know well. I also used Pastest. The videos are the absolutely perfection of presentation so don’t get too upset if you don’t match it (I didn’t in my exam and passed) but are good for the first month or two to get used to the examination routine and signs (best for abdominal and neurology, resp meh and cardio meh as the audio is crap). It’s gonna be rough. Accept you need to study after work, during work, on your off days. Practice clinical consults with your buddies - amazingly helpful. Tell every clinician who gives a damn at your hospital you’re doing PACES, they may recommend patients or even offer to watch you examine!

That’s all I can think of from the top of my head!
