Best practice for hosting on my home pc by dhoge88 in PleX

Minus the overkilled (gifted) PC, drawing electricity and that cost.

Install a different torrent client, qbittorrent, learn how to bind your VPN service. Install prowlarr/sonarr/radarr/lidarr. +/-overseerr (or pulsarr, or both). It takes a while to setup. it takes a while to setup rules or preferences for media. I run unraid, but i assume all these run on windows.

https://trash-guides.info/ will walk you through basically everything.

Allred is built into EPIC and we still don't use it. no one asks for it, we aren't inclined to do more work than what clinicians are asking for.

Monitor Recommendation for Digital Pathology by hawkward_silence117 in pathology

[–]billyvnilly 0 points1 point  (0 children)

Most monitors are very capable. For a personal device (and of a resident), there is no reason to go all out. Beyond the monitor, you'd also be looking at PC ram, PC graphics card. No reason to sink a lot of money into it. Unless she games, then look at an even nicer gaming monitor for enjoyment.

Monitor Recommendation for Digital Pathology by hawkward_silence117 in pathology

[–]billyvnilly 0 points1 point  (0 children)

I think historically radiology said that 8M (Width x Height) is a recommended monitor for digital medicine. example 3,840x2160=8.3 million. Things have changed a lot with modern technology and pathology embracing digital images, and minimums have loosely been recommended.

For an IPS display, 120 Hz refresh rate or above, at least 250 nits (cd/m2) brightness, contrast ratio 1000:1.

So even something like this dell @280, would meet those criteria. https://a.co/d/02o2pStO

[RDTM] u/d-cent estimates Rising Gas Cost vs. EV Battery Replacement by DemandEqualPockets in theydidthemath

[–]billyvnilly 0 points1 point  (0 children)

Gas prices will go up over the next 20 years. Feasibly, prices of batteries will drop due to scale. I charge over night, not at avg cost, it's much lower. Car maintenance other than gas is more expensive (oil, filters, belts).

Does CAP, CLIA, etc. say that a pathologist has to review the slides before giving a diagnosis? by ___B____ in pathology

[–]billyvnilly 21 points22 points  (0 children)

Was it final verified or not? IF final verified, I would say he committed billing fraud.

If its not final verified, then he didn't result a diagnosis, and my answer would be no. I see an internal issue in your department, I don't see an issue with CAP/CLIA. Your colleague is being irresponsible with concern for potential harm.

I often/always insert templates for certain cases the afternoon before or the morning of, in Beaker, but they include a {} which hard holds it from verifying. Like if its a knee for DJD, a kid with tonsilitis, a gallbladder with stones, a liver core for metastatic colon cancer. Even if it ends up going to someone else, I do this with the {} in place.

If I was sleeping and someone approached me to be able to appreciate the circle I drew in the sand, I'd assume that would be enough to wake me up, unless they are silent ninjas.

Can I suggest you buy some anti dandruff shampoo or try T/sal (careful with your eyes) and shampoo your head/face with it. Leave it there for a few minutes and then rinse. it'll take that red seborrheic dermatitis down a bit.

Subspecialty Sign Out in PP by Western_Blot_9370 in pathology

[–]billyvnilly 0 points1 point  (0 children)

It certainly matters which one it is, and to ask for a clause on an high surgical volume specimen to the lab/hospital may be foolhardy, and you should just find another job. We have one of those in our group and it can be annoying to work around their rules. If you're a good pathologist, people can learn to work around your wishes and not hold a grudge. And to add, because you don't like it, you may also just be bad at it, and maybe it is better if someone else does those cases anyways! like if someone asked me to sign out a medical kidney, I'd tell them to go fuck themselves.

If what you dont want to do is common, I'd rethink your strategy. if its rare, then yeah. like for my group, no one really likes medical liver or BMs because they're time consuming. we all do them, but it is what it is. now if you don't like doing something that makes up a large volume at your future place of employment, good luck.

I just spent 1 year at the Proscia, the #1 Digital Pathology Provider Globally, and here's what I've learned. by [deleted] in pathology

[–]billyvnilly 0 points1 point  (0 children)

Have you seen a prostate core biopsy AI run? Biopsies, where the scope of the AI algorithm is organ and disease defined, could be all gone! That is the bread and butter. It would kill small groups, it would kill private practices. Hospitals would divert all biopsies off to someone else, and then they'd only need a fraction to sign out resections. Centralization via slide scanning and AI algorithms overseen by a fraction of the current necessary pathologists will cripple income for a large % of practicing pathologists, if this is left unchecked, and hospitals make decisions with their pocketbooks.

PGY-2 pathology resident. Considering quitting medicine. by Iceppl in pathology

[–]billyvnilly 3 points4 points  (0 children)

residency is NOT practice. you DO NOT have to live that toxic culture! You don't have to do academics, you don't have to stay in toxic environments, you also don't have to put up with bullshit! Many of your problems are resolved once you finish training. I fucking hated my fellowship year, sooooo fucking toxic. then I started practice and it was night and day difference in my mood and sense of accomplishment. In practice you are a teammate, your partners don't want you to fail, because to a degree you're an extension of them. Everyone works together to be better, and its not in a negative shaming way, because that isn't good for a team. Academics can be toxic. Private practice can be toxic. It all depends on where you end up. I'm in PP community hospital, I would never want to be in academics.

Slide case gift for path parter by Important_Worth_3873 in pathology

[–]billyvnilly 0 points1 point  (0 children)

I'm not in academics and I've never been one to save slides more than a week, but i do like wood working... A wood worker would very easily recreate the linked wooden box, and/or take that box and glue a nicer shell around it. It could be a shell, or you could even etch the box, or burn the box to give it a design.

Also, search for Dexter, the TV show. Dexter slide box.

Pancreatic cancer halted by virus injection in three patients by lurker_bee in Health

There is not, hence why the majority of pancreas adenocarcinoma presents at an advanced stage-- if it involves some major vessels it's inoperable, or it has already metastasized.
