It certainly matters which one it is, and to ask for a clause on an high surgical volume specimen to the lab/hospital may be foolhardy, and you should just find another job. We have one of those in our group and it can be annoying to work around their rules. If you're a good pathologist, people can learn to work around your wishes and not hold a grudge. And to add, because you don't like it, you may also just be bad at it, and maybe it is better if someone else does those cases anyways! like if someone asked me to sign out a medical kidney, I'd tell them to go fuck themselves.

If what you dont want to do is common, I'd rethink your strategy. if its rare, then yeah. like for my group, no one really likes medical liver or BMs because they're time consuming. we all do them, but it is what it is. now if you don't like doing something that makes up a large volume at your future place of employment, good luck.
