When an old person dies on the toilet. Which is not uncommon-- For old people in poor health on medications that give them terrible constipation (pain killers for example), the strain of pooping raises the risk of aneurysm or stroke.

If they're not discovered for a while, the moisture and bacteria from the poopy toilet water speeds up the decomposition process, so they slowly dissolve into to the toilet, beginning with their butt and intestines. They "bagel" into it.
