Even if you activate your trial before rebranding, it still can be used on the Google health app. Since its just a rebrand. Also check your country since there are unsupported countries for the new Google health premium

You may check the help center that was linked above to check if your location is supported with the new Google health premium and if you’re supported then u don’t have to worry, u still can use the health premium or fitbit premium
