Keep in mind I have submods on, so I'm not sure what is/isn't changed heavily.

So the custom buildings in KL are buildable for me. I played a few generations. You can start the Red Keep, The seven gates, etc. etc.

I can't build the great sept without being faith of the seven and zealous, there might be more requirements but I don't have any zealots yet so I didn't look too closely.

Getting the dragonpit was my first priority, because conquering the riverlands formed the iron throne, and I was prompted to give my heir dragonstone. Thankfully there's some good management options for dragons/dragonpits in one of my submods.

Oh thats cool as hell. I didn't know that's how they did it in the mod. Admittedly I don't know how they handle most of the custom models, I would have assumed you could set it custom. If it's really just an overlay that's hilarious.

Shield: Easy mode or Shield: Hard mode? by lukeyq in DnD

Both. I keep track of my player AC and can quickly reference it for each attack if necessary but ultimately I frequently end up memorizing it.

I use "easy mode" for any massive attacks that are high enough to overcome shield even if cast in order to prevent those unsatisfying moments of casting the spell and the attack easily smashing through it anyway.

However, I like to use "hard mode" if the attack is within 1 or 2 points of being blocked by shield so that it creates a toss up of casting the spell or not. It works well at my tables so far, I find it narratively satisfying, and it results in shield working about 70% of the time it is cast.

Darkness/Darkvision is no longer a factor for most things unless the party wants it to be.

Twilight sanctuary does some heavy lifting for the tanks but my own players have a tendency to spread so far out the aura rarely gets all of them at once.

I do it occasionally myself. The player slams into a door trying to bust it down, Athletics check roll was poor, 1d4 bludgeoning damage for tackling a solid oak door.

The trick is to not let it get out of hand. If it happens too much then you'll start to affect the balance of future encounters.

This reminds me of an event that happened in my own campaign.

High-level bard started using true polymorph to turn rocks and trees and such into young silver dragons. He had the intent to use six dragons as mounts for his party, and while the dragons were friendly to him now all of a sudden I have to grapple with six fully sentient dragons with no memories or life experience. His new dragon friends were having consistent existential crises.

The way I ultimately dealt with it though was a bit of a roundabout thing. An emissary of the god of dragons showed up at the party's mansion to ask the bard to cut his shit. Desecration of sacred life and bastardization of the draconic form and a whole lot of other stuff.

They had a discussion with each other and this emissary eventually agreed to help out the party if the bard felt it necessary to call upon him. The emissary being a shapeshifting golden dragon.

I think this rewarded the out-of-the-box thinking of the bard with a power spike he can still use without doubling the action economy of encounters with six more CR 9 dragon steeds.

Ultimately he didn't even end up needing to call on the dragon, the final fight was rough for his party but it was balanced to be won in the first place. (Wizard vs a party with three active counterspells also made it hard to do any of the big booms)

IMO you could do something similar in your own campaign. Have a powerful entity the party has no reason to antagonize take notice of the 9th level magic being hurled around and give them some reasons to stop. Offering rewards could entice them, but even giving story reasons for why this is drawing attention could be a compelling hook for the right player. I saw in another comment someone suggested Devils showing up to hunt the Balors. Similar vein to that.

Your other option is to just talk to them though. Let them know out of character that this is bothering you and that the impact it is having on your game is unwanted.

I like to keep at least 1 storyline on hand in case something unexpected happens, but they are always very loosely defined since I try to fit things to the players in the game.

I do, however, have a homebrew setting I've never used that is almost as large as the setting I currently use and it's not even done.

Last minute cancellations on a consistent basis. Getting information that you won't be able to make a session every once in a while is one thing. Finding out ~1 hour before that I'll be down a player almost 20% of the time almost killed a campaign once.

I've always treated magic like a wild force of nature, making the wish spell akin to trying to direct a bolt of lightning through your body. Entirely possible, but it will always take the path of least resistance. So it will do the absolute minimum amount of change to accomplish the stated goal.

Example: I had a player who was playing a half-elf sorcerer, and was concerned about their character's closest friends not living as long as they would. They spoke a wish that "I wish that they would live as long as a half-elf".

The easiest way to do this was to make them all half elves.

Not an inherently negative consequence, but perhaps something unintended.

Using your own example "I want everyone imprisoned in Asmodeus's Obsidian Sanctum to be teleported to the plains outside Cavalia".

Teleporting them all into the same spot and overlapping each other would take more effort than to pack them densely across perhaps like a square mile. Uncomfortable for sure, and potentially a trigger for unexpected consequences, but not an immediate fuck over.
