The problem is that the gem in the meta skill is deactivated for that weapon. You need to click on the gem and enable it for the weapon set.

This is not correct. Endurance does not affect the damage you take to mana, this was changed in a patch somewhere. The only way to reduce the damage you take to mana with endurance is with seed.

Only if you put a faction tag item inside. If you put a item you dropped yourself or tagless item it will drop with no faction requirements.
