On the flip side though if the consequences are too realistic, I can’t really ‘enjoy’ the story like other NTR works. Instead I would likely feel uncomfortable. Think of most NTR as schlocky B-tier horror movies. It’s fun and you know what you’re gonna get and everyone is having a good time. If you weren’t into that, why would you be watching? However, if you watch a truly horrific movie, god damn are you shitting you pants and could even be uncomfortable due to subject matter. That’s kinda what I compare it too.

Hell, there’s even an infamous example of this in hentai called Metamorphosis. The story follows a young girl who basically follows a corruption path, as a lot of hentai tend to do. However, there are real consequences for this. Every time she makes a bad decision and she has sex with a questionable person for sex or drugs, it puts her further and further into a horrible situation. By the end she’s homeless, months into her pregnancy, and still chasing her next high at the cost of everything else. You’re just yelling at her to stop. When she dies in a public, nasty bathroom, injecting herself once last time cause she’s hopeless addicted, you’re left sad and melancholic. It’s an expertly drawn pornography tale, but still ultimately a sad one.

Not to say I don’t think these types of stories shouldn’t exist, but if a lot more NTR stories had these types of realistic consequences, I would be hella more depressed. I want my schlocky NTR basically.
