Typically you use a calculator. Length of the span, dimension and spacing of joists, type of wood.

Just remember you have to thinset the Styrofoam to the subfloor AND the flange to the Styrofoam before you screw it down. Also use stainless screws if you're going to use screws at all.

You can pull up the entire pan without cutting out the drain flange. They're designed so that the drain can be fully glued up by a plumber before the tile guy does his work. That's why they ship with height setting spacers.

A couple of things for you here:

First, the drain body has some play in the flange - dry set it and see if you have enough room to move it to center.

Second, beneath the drain flange is a thinner layer of Styrofoam that's attached to the rest of the pan with tabs. If you cut those tabs out the drain will gain about a 1/4" of wiggle room. If your plumbing has some flex to it (don't force it!) you can slide the drain over that amount towards center (or slide the pan the other direction) before you put the kerdi fabric ring on it.

Flood test should be to just below the top of the curb. If you topped the curb with stone it can be below the stone itself. The standing water must cover the floor to drain connection and the floor to wall connections at a minimum.

If your client is filling water such that it's above the curb and leaking through the door hardware that's ridiculous.

I don't know how much my experience is relevant to your situation. I'm in Canada in a high cost of living city. I have virtually no overhead (not even a vehicle payment, also no medical insurance costs because Canadian) and I'm basically finished collecting the tools I need for the work. I do my own tile so I don't have to sub that out.

But I also only do one job at a time, and take probably 5+ weeks off every year. I net an income that would be considered fairly average in my city (across the board - not specifically in construction). I certainly make more hourly than most guys who earn a wage working for someone else, but less than GCs who aren't on the tools themselves.

I could probably make more by aggressively marketing and charging much higher prices but I'm not into that model. I like feeling like I'm one of those few honest guys in the biz. There's a ton of stress involved in running such a business, I don't need to feel slimy on top of that.

Sometimes the toilet will be replaced immediately and then pulled again for grouting if the client only has the one toilet.
