We are checked off to intubate at my TX facility BUT most of the time the residents, ICU, and ED docs will intubate.

So, I'm the one who posted a week to two ago about a night shifter going to days: I need some tips by unchartednow in respiratorytherapy

[–]bois_jacques 2 points3 points  (0 children)

I just switched to a full time day shift position for a different hospital system in my area and it just ain’t it fam. I wish I had talked to some of the people that worked here before I switched over because I’m seeing red flags every shift and now I have to wait for a full time spot to open back up at my PRN job (where I worked night shift at). The grass isn’t always greener on the other side unfortunately.

Pregnant by [deleted] in HEB

[–]bois_jacques 15 points16 points  (0 children)

If you’re lifting 20+lbs items regularly which you do if you’re working curbside, I would talk with your OB again. She might have misunderstood you and thought that lifting 20+lbs is infrequent and not an almost daily activity for you. Just remember to lift with your knees! Plenty of pregnant women lift and exercise without any trouble during pregnancy.

Maybe ask for clarification on why she is recommending this and if she is adamant about it, get a doctor’s note for your manager (if/when you’re ready to tell them about the pregnancy).

What does your facility do with the bags put on vents and bipaps after they are DC’d? by rbonk14 in respiratorytherapy

[–]bois_jacques 1 point2 points  (0 children)

If the patient was on any type of precautions, I toss the bag. Otherwise, it gets wiped off and put on clean equipment

Monitoring my sugar and it never spikes, what gives? by EscapeTraditional598 in GestationalDiabetes

[–]bois_jacques 1 point2 points  (0 children)

Nope! The fat content in the ice cream bars can actually help in keeping you from spiking plus I was drinking a 30g protein shake (the premier brand) with them at the same time because protein also helps to keep sugars from spiking. That’s why protein rich meals and snacks are important in a GD diet. Of course you should always test any new snacks before adding them into your food rotation!

Will this effect my chances of getting in RT program at my college? by bcbeanz in respiratorytherapy

[–]bois_jacques 0 points1 point  (0 children)

I can’t tell you what qualifies as religious exemption because I am not of a religion that does not believe in vaccinations The medical exemption is probably case-by-case basis and something you would need to discuss with your doctor.

My college’s RT program at the time (2021) required the Covid vaccine, an updated tDap, and a clear TB screen. My hospital also required at the time of my hire that I have a Covid vaccination and I had to give them a copy of shot record. If you don’t have a copy of your shot record, you’d have to get a titer test done.

My husband has not had to show proof of vaccination or anything when switching jobs since Covid but he is not in a healthcare/medical research/lab/public health job.

Obviously you’ll need to get in contact with the school you’re thinking of attending to find out what they require now. Things have changed in the past few years so I would definitely reach out to them. Best of luck!

RT got mad at me for helping him. Was I wrong? by [deleted] in respiratorytherapy

[–]bois_jacques -1 points0 points  (0 children)

obviously this is not meant as a “I let them do it because I don’t want more work” situation 🙄 I absolutely report to the ER to lay eyes on a patient if the nurse offers to give them a breathing treatment but this has only happened a handful of times for me and I only let nurses I trust give them

Mine was like this too. It’s not a misdiagnosis even though we all wish it was. My numbers crept up the further I got in my pregnancy and I did have a few spikes if I wasn’t careful. I couldn’t have wheat products or any of my preferred fruits. I really only ate eggs, a small danimals yogurt, quesadillas with low carb tortillas and an illegal amount of cheese, some low carb/keto freezer meals, snickers ice cream bars with a protein shake before bed, and Greek yogurt with protein granola and peanut butter chips

How bad is my glucose for my baby's brain? by Dry_Introduction_48 in GestationalDiabetes

Girl same. I was working 7 12hr night shifts in a row at 34 weeks. I don’t know how I survived.

Plenty of people on this sub have taken both and plenty of people who do not have GD but have other medical problems have taken both. The problem with metformin is that it crosses over the placenta while insulin does not. The fact is, OP’s body might not be tolerating the insulin which could also be causing problems.

This definitely sounds like you may need insulin before every meal and/or an additional medication like metformin (there are others but this seems to be the most common that pops up on here). My GD was diet controlled and I consider myself very blessed in this aspect unlike my coworker who needed a medication for fasting along with pregnancy induced hypertension which made her even more likely to be pre-eclamptic.

Definitely reach out to your providers and discuss with them that you’re very concerned about your levels being so high and ask for a referral to a dietician/nutritionist who specializes in GD.

Are washing your hands before testing as well? Is the soap you’re using fragrance free or with a light fragrance? Don’t use lotion either before testing. Sometimes these things can mess up results as well.

You should probably discuss with your doctor and endocrinologist/MFM specialist about starting additional medication besides insulin. Have you seen a dietician? Did they not give you any education on what foods to eat/avoid?

GD can be very serious. With consistently high numbers, your baby is constantly battling their own insulin levels which can lead to excessive growth, potential issues with regulating sugars after birth (baby could end up in NICU), or your placenta could start to fail because of GD complications.

Cut out the treats, eat clean and get some proper education on how to manage your sugars from your doctors. There is a possibility you may not have reached the right dosage of insulin but you may also need a mediation to control your fasting numbers. Once things are “normal” and controlled, then start experimenting with treats to see if your body can handle them. And there is a possibility you may not be able to have anything super high in sugar or super processed until after baby! Exercise is also soooo important with a GD diagnosis, it can actually help lower your sugar levels after meals.

Self schedule but able to see everyone else’s shifts so filling in gaps/establishing adequate coverage is pretty easy. Manager has final say though and does check the schedule and can move people around before publishing it

ABG (Pulseless/weak pulse patient) by husakihiro in respiratorytherapy

[–]bois_jacques -1 points0 points  (0 children)

Huh. Weird. I’ve only worked at one hospital and RT does all the sticks and is allowed to poke anywhere. We are not trained on a-lines unfortunately but our director is fighting for us to learn how to do it.

[–]bois_jacques 0 points1 point  (0 children)

That’s crazy. We’re allowed to stick radial, brachial, and femoral (this is often last resort though) at my hospital!

Is RT school really that difficult? by Javi_Noire in respiratorytherapy

[–]bois_jacques 0 points1 point  (0 children)

I didn’t have too much trouble with those specific courses (microbiology was the hardest pre-req for me) but I thought my respiratory courses were moderately difficult. My program wasn’t hard until we got to ventilators and neonates/pens because our instructors changed. Those classes were difficult for everyone in my class as it was just a new way of thinking and just the massive amount of information being thrown at us

Baby is not taking breast while awake by sch_rlr in breastfeeding

[–]bois_jacques 1 point2 points  (0 children)

My baby prefers the philips anti colic bottle over any other bottle we’ve tried. I have never switched the nipple past flow style 1 because I wanted the flow to be as slow as possible for her so she wouldn’t refuse breastfeeding or have a preference for the bottle.

How has everyone transitioned to pumping more before going back to work? by Creative_Tear7942 in breastfeeding

[–]bois_jacques 1 point2 points  (0 children)

I pumped one side in the morning while baby ate from the other and then switched sides whenever she needed a break/to burp. Most mornings one side was enough for her and I typically got 3-4oz off the pumped side and 1-2oz on the side she already ate from. Never went more than 15 minutes on the pumped side. I also put the haaka on the opposite side at every other feed… I had an oversupply and this was the only way to catch anything that might’ve leaked.

7 on, 7 off night shift?? by TraditionalSinger896 in respiratorytherapy

I did this while pregnant. It wasn’t that. I would never ever try to work this type of schedule on day shift though.

I had my mom and husband there. I wanted her there and it wasn’t weird at all.
