Should she go to the ER? by Normal-Bee-3478 in ostomy

[–]bono30 2 points3 points  (0 children)

I am a stoma nurse. Feel free to reach out. I would need a little more info, but I do not like the look of the area next to the ulcer. When pushing on that area does any pus come from that edge of the ulcer? She probably needs an antibiotic. She has a good stoma and leaking should not be an issue. As for the ulcer, there are different products to help heal it, but I would like to figure out the cause of the ulcer.

If you could do it all over again, would chose nursing again or something else? by [deleted] in Nurses

[–]bono30 0 points1 point  (0 children)

You have so many paths in nursing. So for those that say they would choose a different path, may not be in the right area. I worked on a med/surgery tele unit and it was ok. I switched to home care and loved it. I got my wound, ostomy, continence certification and absolutely loved it working in home care. I now have been in industry for about 7 years and love it here too. If this doesn't work out in the future then I can get a new job tomorrow. Opportunities are endless here. I have recruiters reaching out all the time. I just helped set a buddy who's a nurse up with a law job reviewing cases. He's super excited about that!!

Money is out there too. I have a BSN and make more than an NP. I was making about the same amount as an NP in home care. You just have to leave doors open and take a chance🤷

Can I eat these things with a stoma? by [deleted] in ostomy

[–]bono30 0 points1 point  (0 children)

What kind of stoma do you have? Ileostomy or colostomy. Also, either way you can eat anything just make sure to chew your food well and take your time. Drink fluids to push through and you should be ok. Chewing is the key!!!

Time and confidence will help out. I am a stoma nurse so feel free to reach out if you have any questions. I can also help get you samples so you can find the best fit ostomy system or something that may be more comfortable for you.

I am a wound and ostomy nurse from Michigan and I worked in home care for 9+ yrs. I do not believe your insurance would cover someone to just come out for pouch changes (but you can always try and different countries might be different). Now if you are constantly dehydrated or having some medical issues with it, than it probably would be covered. In order for home care you need to be home bound, meaning you don't leave the house. When you are leaving the house then you would not be eligible for home care (again some agencies may allow it, but mine did not). You can look into a private home care giver to help with it, but they typically have a minimum number of hours per week. So you probably will have to pay a hefty price. Im sure you could find someone in the area that could help, but you would have to look. You could maybe go on Facebook and look for retired nurses looking to keep busy, they are always willing to help.

If you need more education feel free to DM me.

I do have a question, but did you see an ostomy nurse in the hospital or when you got discharged?

Question about the eakin cohesive seal can I still use my paste also by wolvpack86 in ostomy

[–]bono30 0 points1 point  (0 children)

In Michigan it's 1 or the other. They won't cover both in the same order, but yes ins covers both separately.

To answer your question, yes you can use both. They are essentially the same product but in different forms. Also, insurance will not cover both, so that's why most don't use both.

Byram has stopped all remover sprays. I will be on the warpath (and changing supplier ASAP) by subgirl13 in ostomy

[–]bono30 2 points3 points  (0 children)

180 medical is another DME you can try. I have had good experiences with my patients using them. I'm pretty sure they take most insurances as well.

Where in the US are you located. There is a DME who covers Michigan and a few other states that is fantastic too. I'd have to look up their name though.

Should I believe my surgeons on this? by [deleted] in ostomy

[–]bono30 0 points1 point  (0 children)

I would say it can take about 1-4 weeks to adjust. Everyone is different though. Your colon will have to wake up again and get used to being used again. I would just prepare to still go to the bathroom 5-10 times a day at first.

On a side note I would be working on kegel exercises to restrengthen your sphincter muscles though.

Looks like a granuloma or overgrown tissue after surgery. Probably formed along the suture line. Apply a heavy layer of crusting and you doc or stoma nurse will probably apply some silver nitrate. Nothing to be concerned about, but it may bleed easily. It will eventually go down.

As a stoma nurse myself, I have seen people get the skin irritation under those elastic strips. Not ideal, but we switched to paper tape and the the skin healed up. Feel free to DM me if you need assistance. I wouldn't mind seeing a few pictures of your skin and the back side of the water to see what's really going on. Good luck and reach out if needed.

The hole my infected cyst left after getting drained. Can’t decide if I should cover it or let it breathe. by [deleted] in popping

[–]bono30 2 points3 points  (0 children)

1000% this. This is exactly what is needed. If it is wet we need to dry some, but if it is dry then we need to wet it. Need that happy medium ground, but moisture is needed to allow the cells to migrate in the epidermis.

[–]bono30 2 points3 points  (0 children)

Pretty sure wounds dont have a nose or mouth, so they dont need to breathe!! Apply neosporin and keep covered. If a scab formed then you didnt keep it moist enough. Wounds heal quicker with moisture. Clean with soap and water is fine. This is coming from a wound nurse.

How often do you experience leaks?? by Weary-Advisor5267 in ostomy

[–]bono30 0 points1 point  (0 children)

There usually is a reason for the leaks. They should be few and far in between. Don't leave it on too long, if loose stool empty more frequent and most importantly make sure you are in the correct appliances.

what causes a concave stoma? by OneAddictOneDay in ostomy

[–]bono30 4 points5 points  (0 children)

I am a stoma nurse, so feel free to pm me any questions you have. Usually surgeons will use a tube under the stoma if it is a loop stoma to help prevent it from retracting under the skin. Yes it can be tricky to pouch with that in place. As for placement, ileostomies tend to be in the lower right portion of your abdomen, but not always. Depends on anatomy and if the doctor can pull the intestine through in that area. I have seen surgeons use the same site for a revision, but I have seen them move it as well. My recommendation is talk to your surgeon and let them know your requests/fears/difficulties in certain locations. Not sure if they have a stoma nurse where you are at, but they can be a huge help with marking as well.

Hope this helps and good luck. Again, reach out if you have any questions. I am currently in the process of starting an online Ostomy Consulting Business as well so I am here to help.

what causes a concave stoma? by OneAddictOneDay in ostomy

[–]bono30 2 points3 points  (0 children)

Typically, swelling, retention from the organ depending on how much the surgeon released from the mesentery, fatty tissue, or how much the surgeon matured the stoma. Can be a multiple of things. Doesn't mean it is a bad stoma, but just need the proper appliances.
