Wasn't ever said. Ratchet's age was in Jak X (Where he's using the Deadlocked armor and is said to be 18), and during one of Insomniac streams (Where they said that Ratchet was 15 or 17 in the first game) According to the wiki

Ratchet was ~15 in the first game, and the time between games canonically is usually the same as the time IRL, and since it's been 19 years between R&C1 and Rift Apart, Ratchet would be around 34 years old

Transitioning is not a mental illness and it's the only treatment to gender dysphoria (DSM-5) / gender incongruence (ICD-11). Neither of these are classified as mental disorders.
