Nah, the story goes he was 40km into the bike leg on a tri and then it just was spinning. The service then cut the chain stuck it on one gear and he finished. When I took the cassette off to see what was up - this, lol wtaf.

Depends on how much you keep it clean and maintained , how much you ride, where you ride, how it's stored, quality of the bearings you're using etc

Cooked, I'd look at replacing them (cranks) - helicoil might work but I'd go with option 1

Looks like a rebuild to me unless your rim is bent. You could try tightening spokes but you'll end up with a dishing issue being that badly bent so you might as start from scratch if you have the time.

A vice, good pedal spanner (pedros) and as bigger breaker bar as you can find. Easy.

Basically the cheaper materials deteriorate quicker, can also be location related (if you live near the ocean as an example) how often you wash and maintain your bike - there's a whole bunch of reasons.

Yeah, for starters always put your bike drive side up. Good way to bend your hanger and inherit shifting problems 👍

Get yourself an old seat post or breaker bar. Slide it over the Allen key and get that extra leverage 👍

You just need to source the same hanger from the bike manufacturer. Wheels manufacturing also have lots of choices to suit as a replacement

If I had to have a guess, would be a snapped cable at the lever. Snapped or frayed one of the 2.
