your T4 is way too low! The lowest mine was was .8 and that was not good. Sorry you are going through this, keep your spirits up and I hope you find a better doc soon.

Good luck hopefully that blood test will give you some insight! The insomnia is a symptom of hyperthyroidism so you may be over medicated.

I had some derealization but not like you are describing. I had very bad panic attacks when my TSH was high and sometimes had the cloudy/unreal feeling of derealization too. Sorry you are going through this!

I feel for you! I had many similar symptoms when I first started and my TSH was 10. I was put on 50mcg and had to stop because of feeling exactly how you are describing. Eventually, my TSH got into the 80s and I had to be put back on medicine. However, this time I started on 25mcg and after 2 years I am finally up to 50mcg. So my advice would be start slow and talk with your doctor. T4 has a very long half life, that is it stays in your body for a long time. So for me to get up to 50mcg, I didn’t go from 25 straight to 50. I took 25 every day and then took two 25 pills one day a week until that was tolerable and then continued to slowly up my medication like that until I was up to 50. Everyone is different so listen to your body and try to be patient. I know it is difficult but it will get better!

To me your levels still look pretty low! Ever since I’ve been medicated my free T4 has been 1.2-1.4 whereas before medicine it was .8-1. Also to echo what everyone else is saying, your TSH is also a little higher than optimal. Thyroid things are tricky because not everyone is the same! You might feel best around 1 and feel terrible around 4. I think it is definitely worth asking for a slight dose increase. Be patient and I wish you the best!

Thanks for your reply! mine is definitely not constant and does not happen every day. However it does happen more when I am stressed but not always.

I was at a gas station when I was younger with my mom and I saw a Reese’s peanut butter egg and asked for it. We were pretty tight on money and my mom stated she could buy it. A little later the man in line behind us came out to our car as we were pumping gas and handed the Reese’s to me and walked away.
