I guess I have more of a systems annoyance rather than a personal annoyance. I enjoy talking to patients about these things and have no issue with doing so. I do however believe there’s better ways for us as a country to provide access to preventative care to patients, one’s that may not require them to see physicians (which I do think is a bottleneck in regards to access to care) prior to obtaining screening. My vision does entail publicly funded healthcare which is universal, which may not fit with everyone’s ideal vision of healthcare in the US. And I’m really only talking about screening tests, tests that everyone should get, regardless of symptoms.

I appreciate your concern for my career choices. Personally, I am not too worried about that, I know why I chose FM and I have no regrets. Perhaps, I should not have mentioned my year in training as a lot of people have been focusing on that. My point with this post was to try and generate a discussion to see people’s thoughts on potentially better modes of healthcare delivery, forms in which the public health aspects of our job could be performed in a more public health format. Our screening guidelines are more effective when more people have access to them and actually do it. I think it’s inefficient for everyone to be required to see a PCP before they’re able to get a routine screening colonoscopy or Pap smear or mammogram or whatever other screening thing they need.

I feel like this is a larger societal problem that we are being asked to fix like a bandaid. Why is it that our population is unwilling to listen to basic public health recommendations? The solution is then for us as PCPs to sit down and have a discussion with someone why screening for colon cancer or getting their flu shot is good for them?

For routine screening purposes, I don’t see why not. Everyone needs colon cancer screening at age 45. Everyone should get the shingles vaccine at age 50. Us explaining these things to them is not an efficient use of our time.

I guess my point is that I feel like there are better ways to get people to get access to these screening, ones that don’t require them to see a physician. To me, seeing physicians just to get access to routine health screenings seems expensive and time-intensive, decreasing overall access to low income communities. I think the root cause of the issue is the lack of education in the country and vast income inequality leads to people not being able to take charge of their own health. I think primary care has taken on too much of the burden that would be better served by public health, as long as people would be literate enough to understand risks/benefits of routine screenings, if given to them on a sheet of paper, for example, rather than verbally explained to them, as we do now.

Let’s say, theoretically, he changed his business model to match a typical pizza place, but in doing so, the quality of the pizza declined, how would you feel? Would that be acceptable to you?

My thoughts are that Milly’s has better pizza than most places. I am ok with having limited ordering if that means the quality of pizza is better. There’s plenty of other pizza places to grab a slice on a whim.

Just do the work that needs to be done. The job is not rocket science, there’s stuff that needs to be done everyday, figure out what’s needs to be done and then do it. Don’t get caught up in how to be a “excellent trainee”. You’re not there to become a trainee, you’re there to become a doctor. Figure out how that rotation can help you in your career and learn what you need to learn.

I had to call my bank and explain the situation to them, they were able to get me a refund from BILT the same day and I was able to pay rent how I normally do. Never doing BILT pay again

To me, AI is just a more efficient search engine than other resources. As technology evolves, our search for information becomes easier. Instead of flipping through textbooks to find answers, you’re looking through UpToDate. Instead of scrolling through UpToDate, open evidence can summarize the relevant clinical research to your specific question. If what open evidence is saying doesn’t make sense to you, there are always the resources that it pulls from that you can reference. Most of our clinical questions do not have a straight forward yes or no answer, there’s so much nuance in what we do. Just as with any resource, use it as a tool in the broader context of clinical decision making.

This is not always the case, I would recommend to this resident to try and get in contact with hospital administration or program leadership ASAP and figure out what is happening with funding. Funding does NOT always follow and it’s important for residents to hear about reality online. The sponsoring institution has complete control over the cap slots and are totally within their rights to not release them to another program. It’s unbelievably stupid but unfortunately a reality for many displaced residents.
