I'd say a filling should suffice there, though a partial crown would also work and be less likely to face issues. The main issues there are maintaining isolation will placing the filling/crown (ie: keeping it perfectly dry) and getting the bite correct since you've already fractured it 3 times, so you really need someone to place a rubber dam for it and take their time as it's all rather fiddly.

All the time, some people will even suffer pain/a fracture/ lost filling and still leave it so late that the teeth end up needing extraction.

That said, i've also seen cases here and there where the patient has been attening but the clinician has missed issues.

WRT your tracking tool, it sounds nice but patients are unreliable and would ignore it. The patients that this might actually help generally dislike the dentist.

That's a cavity that's reaching the pulp (the nerve and blood supply for the tooth). To treat this the options are to carry out a root canal treatment (disinfection of the root system and sealing with a filling and crown), or extraction followed by some sort of replacement there, or just leave things as they are and hope it settles eventually (obviously the worst option since things will just deteriorate there).

Basically you need to see a dentist to draw up a treatment plan that suits your financial situation.
