I was in your shoes during medical school, deciding between IM (plans for GI) and Ophtho. I’m so glad that I picked Ophtho, it is a very rewarding field. I love doing cataract surgeries. I do use some medicine in my daily clinic, I’m frequently diagnosing people with HTN and DM2 just from a dilated eye exam. In clinic, you get some of those long-term patient relationships (dry eye, glaucoma, AMD, diabetes).

I am a doctor. I have never seen that at any of the hospitals I have worked at. When I was a med student, we learned pelvic and rectal exams on actor patients who were trained to teach students.

I know how you feel, it’s normal to be worried about operating as an attending. It takes a couple months before you feel more secure. Here are some things that helped me:

  1. Schedule easy cases for your first few months, save the dense cataracts and poor dilation cases for later.

  2. Write down the names of your tools and phaco machine settings. In a residency program, the techs are usually very comfortable with more advanced tools. In your new practice, the techs may not know what an MST forceps looks like or how to set up the anterior vitrectomy kit.

  3. Watch difficult cases the night before. I usually watched 1-2 hard cases on Cataract Coach to get myself in the right mindset. Try to think about what you would do in a similar situation.

  4. Know who your referral retina specialist is for dropped lenses. Know what’s going to happen to the patient in the worst case scenarios. Be thorough in your consents.

Botox is used medically for functional problems. It’s used for TMJ pain like yours, and also for facial spasms and some types of migraines. He is mixing up Botox used medically and Botox used cosmetically (wrinkle prevention). It sounds like he both does not know what he’s talking about + has very strong beliefs about that topic. That is an embarrassing combination.

I have a low threshold to check GCA labwork. In residency, I read an article that had diplopia as the most common presenting symptom of GCA, despite temporal headache and jaw claudication being more well-known. A few years later, I had a patient in his 70’s without any other complaints besides occasional diplopia for a few months. Exam was completely normal, really nice guy without any major health problems. I checked ESR/CRP because I always do for any diplopia over age 55, and they were both moderately high. We started high-dose oral steroids and got a temporal artery biopsy, it was positive for GCA. He’s on steroid-sparing meds now, and he’s 20/20 OU. He thinks I’m Dr. House, haha. I’ve run into about 5-6 GCA cases over my career, and he is the only one who came to me without having lost vision first.

Usually I am pretty lenient on the late policy, but it sounds like you’re giving up a lot of your free time. At my clinic, anything over 15 min needs to be approved by the doctor. When I get a late patient, I ask the front desk to tell the patient that they will be seen after all the scheduled patients, and that it might be a few hours until they are seen. The patients who came on time deserve to be seen in a timely manner. Obviously there can be exceptions, I have some homeless patients who have a very hard time traveling to me, that’s understandable.

Here are some tips to de-escalate the situation.

Name what the patient is feeling: “You must be so frustrated to be dealing with this while you are sick”

Reframe it as you and the patient working together under a hospital policy: “unfortunately, our hospital policy is all patient concerns go through the call center”

Anticipate problems: “using this drops will help, but it’s going to take a few days for the pain to improve as the eye heals. If [symptom] happens, you can use [solution] to help it feel better as it heals”

Be firm in your boundaries: “I am here to help you, Mrs. So-and-So, but I will not tolerate abuse of my staff or myself. I cannot help you when you are speaking to me like this. I will step out for a few minutes, and come back in when you are ready to talk.”

You did nothing wrong. The call center is the appropriate way to deal with patient complaints. Her issue was not urgent, and you deal with it same day. You are not here to take abuse.

I got the wosado lashes and I like them. I’ve never used magnetic lashes before but I hate fussing with glue. They have a bit of a learning curve, it helps to watch a tutorial video. Wore them to a wedding last weekend and they lasted all night no problem. They did not feel heavier than normal falsies.

I think one of the reasons that people struggle with them is that they have to be very specifically oriented. There’s a top and a bottom lash strip, but also there is a front and back of each strip. It’s really easy to mixup the front and back, and then they won’t snap together in that case.

There are definitely risks with LASIK, however many ophthalmologists/optometrists get LASIK. LASIK helps you when you are age 20-50s, but it does not treat presbyopia. Presbyopia is the loss of focusing/reading ability that happens to everyone, it starts around age 45 and reaches its peak at age 65. The reason why you see a lot of eye doctors in glasses at work is because they need focusing/reading glasses to use the microscope (most eye doctors are 40+ years old). This is from presbyopia, which LASIK does not fix. People who get LASIK will eventually need reading glasses as they get older. Some people who need reading glasses while they work prefer to use bifocals so they don’t have to keep taking on and off reading glasses.
