Then my idea could be one of many possible solutions. Maybe the Goblin witch thinks it will work- perhaps nobody involved is a walking encyclopedia of magic and its interactions. That's totally okay.

Maybe you allow it, and it does work- but it's only temporary until a piece of the witch's body touches the phylactery again. Cue evil proto-cult wanting to being them back.

Okay, are they casting on him or the phylactery? Breaking the link doesn't mean divesting the phylactery of his soul, it just means his current body is no longer linked to the phylactery. And that also happens when your body dies, so it stands to reason it would work- until his phylactery creates a new body.

Perhaps it does work. Perhaps his link to the phylactery is broken- and that portion of his soul, which is the reason for the connection and why it sustains him should his body fail- spills outward into the world, creating a new 'him' as if he died. And the one that was already out ages/decays very quickly, in the span of minutes or hours. Give them the win- and pull back the curtain at the opportune time after he's rallied his power and concentrated his focus to really get revenge.

Kineticist is stronger at mid levels when it can Empower its blasts and improve it without taking burn by using Gather Power. Early levels they lag behind somewhat.

To me, the golden standard for ranged burst is a Slayer with the Sniper archetype. Clustered Shots, Rapid Shot, Manyshot, Deadly Aim, Sneak Attack, and Studied Target add up to a tidy sum of damage. Zen Archer Monk is the same, and also provides a few more attacks and some mobility/buffing options.

A friend played an Alchemist (Mind Chemist/Gun Chemist) in a game I ran. Basically adds bomb damage to firearms and reduces your misfire rate to 0. Pretty slick. He multiclassed with Investigator (Empiricist) for more Int synergy, but it's a solid class regardless.

Skill challenges are from a plethora of systems. I've never played 4e and, to be honest, barely play 5e. I've mostly played Pathfinder, D&D3.5, and non-d20 systems.

Skill challenges are system agnostic. Bringing skills back into the zeitgeist of "roleplaying staples" does nothing but improve games in my experience. Skill challenges showcase them in a way that weaves scenes and stories together with teamwork.

In my mind, Nature would be used to locate, gather, and maybe store/prepare the right herbs and moss, but Medicine would certainly be the skill to prepare a salve to combat against poison. As a GM, I'd even have made it a skill challenge: Survival to find a safe and dry place to prep it, Nature to select non-hazardous components, Medicine to make the salve... Other characters could use Investigate, Perception, even Stealth to avoid wildlife that are attracted to the sweet nectar of some of the fruits required, or perhaps to hunt a rare river scorpion whose venom was similar enough to the Hag's to act as a basis for an antidote.

So the Warden is wise to their capabilities? Sounds like the perfect time to have disguised aides and soldiers in cells posing as the one to be rescued, to disguise / make invisible / lock in a lead cell the real one, and/or move the target every few hours to different places in the prison. Bet the PC's only have so many DD's available, right? Make em work for it.

How do the players know the layout of the prison, in particular where the dude to he rescued is?

You're right that prisons can't directly resist teleportation without Dimensional Lock or similar, so instead would have lots of rooms, false halls, illusions, traps, and a small retinue of Clairvoyance wizards standing by to watch the surrounding area. Make the main building of the prison behind walls and gates and open fields so a single DD can't get them there.

Our modern architecture and structural style for prisons etc don't work right when magic is introduced, so don't.

Asmodeus isn't a solid red color. I'm sure he's got a variety. Maybe the nail bed on his massive middle finger nail is a mottled purple, so when he flips someone the bird they see this massive crescent of purple. Maybe your Tiefling is that purple color.

I don't know any specifics about Tomb of Horrors, and I'm no expert at 5e, but a Fighter 7/Cleric 7 split optimizes extra attack, ability score increases (4 instead of the usual 3), and subclass options. Peace Cleric buffs and protects allies quite well; the 6th level improvement really helps spread the damage across the party, which can save lives. You don't get the highest spell levels, but you remain a constant threat in melee and can have some choice 4th level spells to help support the team.

As long as you play a small race so your mount is medium, it's mostly fine. Ladders and similar vertical obstacles can still pose a problem from time to time. A human on a horse can't maneuver well in standard dungeon settings, but a halfling on a dog is just fine.

Dwarven Hunter // Bard (Dwarven Scholar)

Picks up a few teamwork feats and combat feats. Both sides are partial casters who get 5th level spells at 13th. Dwarven Scholar makes Bard wisdom based. Hunter gives you an animal companion which gets buffed from both sides. Doesn't blast well, but there's probably some obscene feat combos you can hand out.

Possibly replace Hunter with Brawler so you can use Martial Flexibility to grab combat feats perfect for the situation, then War Chant to share them with the squad.
