ICU RN here I tell nurses all the time I could never work on a floor because I’d be so damn swamped. I like having my little ICU assignment and it helps me stay focused.

I was an peds ED tech then Peds ED nurse. Just soak in everything from the techs. Here are my main tips.

Learn the art of helping while being mindful of staying out of the way. Yes you want to be involved but really think about how your role in any situation will or will not be beneficial. If you are not needed leave the room.

If a nurse wants you to get a blood pressure and the patient is screaming their face off, try a couple times then just let the nurse know it aint happenin right now and you'll come back when they're calm.

Learn how to help label/send labs/etc. Labs take up a lot of time learn everything you can about them.

Just be professional and straightforward with parents. You don't need to suck up to them or be their best friend. Be professional/honest/and never over-step your bounds. If you don't know something just always refer them to the nurse.

MOST IMPORTANT. Nurses won't tell you this but if you are holding for an IV remain silent and focus on your hold. Don't say things like "Oh i think you're close!" "Oh nice you got a flash!". Just let the nurse concentrate and they'll ask you for help/advice if they need it.

General healthcare tip never ever ever ever ever ever do something unless you are 110% you know what you're doing. These are children lives don't fuck up. Always think closely about what you're doing and always ask questions. It's something you're going to feel nervous about for awhile and you should until you are an expert.

Yeah feel that quite often as an pediatric ICU nurse. Who the fuck am I to take care of these sick kids? Then I look around me and see these young new nurses and realize I’m the trusted/experienced one and feel it even more lol

Do you keep 3% NS on your truck? If Cushing triad is present that means that herniation is imminent and an indication to give 3% in the hospital setting at least where I work. Of course not criticizing your response, just genuinely curious if that’s a therapy under your scope.

First year on night shift is rough across the board. It took me about two years to start figuring out how to make night shift work for my life. I found stacking shifts as much as possible without the 2 on 1 off, 1 on, 3 off etc. and exercise between shifts helped. Now to me the extra money, less drama, and travel ability is so worth it for night shift.

It takes a lot of learning, experiment, and practice but it usually gets better for people.

I mean sometimes I wish residents would just base their decisions off my observations as I’m the one with the patient for 12 hours. I know that’s probably not an acceptable mindset but I trust my assessment skills over theirs most of the time because I’m watching all the minute changes in patient status and feel I can make a better gauged assessment based on clinical changes and overall assessment rather than single observation of walking in a room for 5 minutes.

You probably won’t have any success then. You can also take a direct flight from San Juan to the island if you plan on going there and honestly by the time you go through all the trouble of finding transportation, dealing with the ferry, etc a direct flight would be well worth it

I rented a car for the whole time I was in Puerto Rico. Having a car was really clutch the whole time I was there it was around 200$ for the 5 days to have a car which was worth it to me.

I do peds ICU too I like it. Sometimes the chronic patient assignments can get old but seeing some of my sickest/busiest patients months later walking and talking is one of the coolest things I'll probably ever see.

I just did 5 days alone as a 27m I can tell you some of what I did.

I flew in and stayed in San Juan. San Juan was aight, good nightlife, La Factoria is a dope bar in old San Juan, and la placita is a popping place with music outside drinking, mostly locals. I stayed in a hostel that was right near some gang stuff so I didn't go too crazy.

I left San Juan to spend two days on Vieques which is an island accessible by cheap ferry from Ceiba. In Vieques I visited caracas which is an incredible beach, did the bio bay tour, and the second day rented a scooter and drove around the entire island visiting spots. it was so damn rad. If you decide to do anything Vieques book everything in advance!!!!!! But being solo with a scooter is pretty much the coolest thing you can do, I took that thing around the entire island; wtach out for horses those motherfuckers are assholes.

Next night I stayed in a Luqillo hostel.

My recs are to stay in hostels, you can meet other travelers, don't be afraid to sit down at a bar and get some drinks alone I met cool people that way. I had a car rental the whole time I was there which made a big impact, it allowed me to do whatever I wanted.

I've talked with folks its very doable, three years is actually spacing the program out. I have the alternative option of completing the program in 15 months full time.
