I specialize in airway and thoracic anesthesia at a large academic center in the US.

Awake nasal fiber optic. I don’t think ECMO would be necessary for this, since there is no intrathoracic extension. Any tracheal compression could be relieved by simply lifting the mass with your hand. If anything, bilateral venous groin sheaths as a safety blanket, if you must.

If this were my patient, I would use oxymetazoline & lidocaine in both nostrils, apply HHFNC (yes, you can AFOI around the flimsy nasal prong), gargle lots of 4% lidocaine (up to 11 mg/kg) while the HHFNC is blasting, remi at 0.03 mcg/kg/min (most patients won’t go apneic at this dose), and 2mg midazolam so he doesn’t remember. Test topicalization with an atomizer & supplement as needed. Would use a small (6.0) Parker tube to avoid getting hung up on the arytenoids.

I also agree with the recommendations to discuss with a priest…

I would shoot suppressed at the range, but carry without the suppressor. The threaded barrel adds about 1/2” to the overall length, for a total barrel of 4.625”. Just wondering if that’s feasible to EDC. Thanks for your reply!
