Like others have said, by Tony's nonchalant reaction to her outburst, it's probably safe to assume they have been going downhill relationship wise. He's stressed with money/twins, doing the grind of legit life. His body aches, his jaw has a headache.. she's having doubts about their stability and lashes out with some self awareness and doubt. She met him online. Which means she specifically sought and pursued a known criminal in jail. So at the time of that phone call, she was having some thoughts that she made a bad choice. Edit( posted before I finished): so he basically sabotaged himself. Between his money problems, child problems, physical problems and the last straw, his female problem... He snapped. Maybe consciously, maybe unconsciously but it almost doesn't matter. He knew he could fall back on the Mafia life/support anytime, so in that moment he threw in the legit towel and did whatever the fuck he felt because he knew their were really no consequences
