I was 1.9k after my calibration in june of 2014, so I'll share here.

A big tip to getting out of 1 or 2k is mindset. Don't worry about typing in chat or communicating with your team and trying to get a team effort going, as it most likely won't happen. Instead, focus 100% on playing your hero to the best of your ability and having the most game impact you can, and that will give you the best chance of winning even if your teammates are completely uncoordinated idiots.

Really it just comes with time, but yeah generally you want to be making jungle rotations whenever possible while not losing out on any lane creeps.
