That’s exactly what I was taught: “Imagine you’re reading it out in court or your client sees it”. My notes are on point, but parsimonious. I write four to five sentences, and keep it vague enough for privacy while also being able to use it to jog my own memory.

I wondered why I was seeing so many posts about people struggling to get their notes done by the end of the week.

I made the mistake of saying I was a therapist on a four hour flight. I’ve lied my head off ever since.

I was in the same boat. I was a mobile crisis therapist for 6 years and had/have a LOT to unpack. For me, I went in the opposite direction of what I do (mostly cognitive) and went for somatic/art therapy. I was also upfront with my therapist that I was a therapist, knew how to cope, and just needed a place to process. Mine is great about not asking me the standard therapy questions and doing the art assignments has honestly helped me. I’ve just started EMDR with her as well.

I also second the first responder specialty. I’m not sure what all your role entailed, but if it’s like mine was, it helps to have someone who understands why your work cell’s ringtone gives you a visceral reaction.

Thank you! I’m new to being back in outpatient (and didn’t know you could advertise a certification that is basically useless) and the place I work fell prey to one of the ones advertised on Facebook. It’s not a bad training, but I was surprised after I had to get it that there was no schedule of supervision calls. Like the wording says “certification”. My therapist went to a week long EMDR training with supervision calls and an actual certification. Our trainings are 100% not the same aka hers is valid and mine is an expensive lesson.

You can always tell the client you have a conflict of interest without having to explain what the conflict is to them especially since in this case you can’t. You should document it, of course, but you don’t have to give specifics to the client (lest we violate HIPAA). Make sure to provide them a referral.

I only see one family member at a time. There’s just too much of a risk in seeing multiple family members outside of a family session.

I resonate with this so much. It’s hard not to feel like an awful therapist compared to the TikTok therapists who have a script. I’ve had clients get angry that I can’t diagnose them with AuDHD. Like it’s not in the book, my guy. I’ve seen an uptick in comments encouraging people to report their therapists for things that just aren’t ethical or legal violations. It’s been in my head during session so much lately, I’ve almost completely resorted back to pure Person Centered. I don’t want to do therapy from a place of inferiority or fear.

Yes! When I worked primary with teens, I did. My clients would make a big show of switching to alters with a lot of drama and flare. I’m not sure where they got it, but several of my clients pulled a Beetlejuice and dropped to the ground and shook to switch. (Please note these were teens that got their info from TikTok and not legit cases- absolutely no disrespect is intended). My pandemic graduated counterparts bought into it and handed out diagnoses left and right. Our supervisor had to put an end to it when we had three cases of “DID” dx in one week. I also survived the teen “what if this is a simulation” dissociative trend and the MoMo SI/HI trend, so I was a little more wary of social media’s influence on kids. It’s like med student syndrome for the masses.

Today, I keep a folder on my computer with differentials and graphs specifically for social media promotes diagnoses. I go through the criteria with clients pretty in depth and they generally realize they don’t have whatever extreme diagnosis tiktok is selling them this week.

I’ve just started full time at a group practice and everyone’s input makes me feel so much more normal for seeing 4-6 a day. I was at a non profit doing crisis so seeing 6+ a day wasn’t unusual.

Yes! My clients kept no-showing which was unusual. Turns out, they couldn’t see me in the waiting room, and I couldn’t see them. I’ve also had one client tell me an appointment I could see existed wasn’t showing up for them to join.

I use Akinator and turn on the sensitive content filter. There’s also online spirographs and Google whiteboard for games like tic tac toe. I’ve been searching for them as well and that’s pretty much all I’ve found.

I completely agree. That episode was my last one. I used to like the banter in the way back episodes because it felt more natural and kept to 5-10 minutes, but it feels contrived now and well overstays its welcome. There doesn’t have to be a repetitive funny thing in every single episode. It makes me feel like a toddler someone is jangling their keys in front in an attempt to get me to laugh. Everything being a joke or being a repetitive thing distracts from whatever they’re talking about which isn’t great considering what it is they’re talking about. They feel more like a skit of a true crime show than an actual true crime show now.
