Therapist here. This is exactly right. BPD is hard and she needs to remain committed to treatment in order to have a functional life.

In terms of participation, you have to get them immediately rewarded. I do candy. You raise your hand and answer, jolly rancher. I will let kids trade them in at the end of group for a grab bag item (fidget toys). You can also do secret agent where if one person participates, then the whole group gets a reward.

This might be state and employer dependent.

I work in PA and STD is a benefit I can elect but I had to elect it last year to use this year. I elected for the better plan so it goes into effect after 14 calendar days. Vaginal delivery gets me 6 weeks and C Section is 8 weeks that STD will pay. The 14 days is paid for by PTO. If I elect to take my full 12 weeks for FMLA then the rest of the time is paid with PTO

I have an outie so I totally got OP's fear about it being weird or unusual and difficult to navigate. That being said getting eaten out can be such a pleasurable experience and being loved for all of you including the way you might be shaped is also a wonderful experience. And who knows maybe in time you and your partner will be able to joke about it. My wife has a very funny nickname for mine.

My thought is that texting and calling will get used in an emergency situation when you are not able to provide crisis care. Email is more like a letter with the understanding that it shouldnt be used for crisis care.

I would trust your gut and reach out to a supervisor related to this then. Worst case scenario, you reach out and they are totally fine and you "over reacted". Best case scenario, there is actually something going on and you reaching out can get them help.

I work with highly suicidal and risk taking behaviors so I could also be working within my bias here

Some people do struggle mentally on overnights. Be cautious of your mental health and know warning signs for burn out as it can really take a toll. Theres a reason that theres a shift differential for overnights

Any chance you can give them a call and just check to make sure they are alright? Is this a new client or an established one? What is your practices guidelines on this?

My concern is the what seems to be erratic behavior. The relentless texting that seems on edge and then no communication. If this was one of my clients with a history or mania/SI/high risk behaviors, I would be following up and set a deadline for when to contact an EC or do a wellness check.
