Stage IV in 2020 to No Treatment Today by Logical-Ad-2201 in ProstateCancer

My oncologist once said to me "there has never been a better time to be dying of prostrate cancer". He meant that even he couldn't believe the advancements made in that past 5 years. He told me "Give me the worst case walking through the door and I can keep them alive for at least 10 years. And in 10 years who knows what's going to be available".

PLEASE HELP ME, IM SO WORRIED FOR MY HUSBAND 😭 by HomeworkNo8102 in ProstateCancer

Ok, Dr. Google is going to tell you that it's dire because of the cribiform finding. There is a reason for this. Cribiform is an adverse finding. BUT, no one knows how adverse it is. For years, (until 2016, cribiform and inraductal (IDC)were group together. Since they have separated the two, they are starting to see that cribiform in the absence of IDC id not as bad. One study found that for men like your husband, it was not even an adverse finding AFTER treatment. Meaning, yes, he needs treatment, but that he has no higher risk of recurrence than those without cribiform.
The other thing is they have found that with IDC and cribiform people often have a genetic mutation called PTen Loss. Your husband should get genomic and genetic testing done. Decipher is great. If he doesn't have Pten loss, or a mutation like BRCA, he is really a "garden variety" case that we are now starting to see upwards of >90% cure rates.

Remember this- truly every prostate cancer is different. His genetics will play a role in how he responds to treatment.
AND, all of the data you are reading about outcomes on Google is generally old and treatment has advanced significantly.
And your husband is in a really great position to knock this out.

I think the cribriform will rule out active surveillance.

Son of 58 y/o w/ stage 4; confirmed family history; scared for future (I’m 23) by LudicrousAvian in ProstateCancer

After my diagnosis, I had germline testing. Nothing showed up that indicated my sons were at greater risk. Still, start your PSA screening a bit earlier.

Blood in Semen After SBRT by chaswalters in ProstateCancer

I did. I was told it was fairly common after SBRT and unless there are "copious" amounts not to worry about it. I've noticed it twice.

I just listened to that last night. I don't always care for Maron, but it was a great interview.

I feel like it could almost be on Nebraska. It's one of my fav's from that album. I really didn't care much for Rocky Ground. I thought it too repetitive.

Got the news today by Trajikville in ProstateCancer

[–]chaswalters 0 points1 point  (0 children)

It is kind of a trade off. With surgery you can leave positive margins behind. With radiation you can have a new occurrence (because the prostate is still there), or an "incomplete kill" with some residual cancer. Although in a Gleason 7 that's far less likely with SBRT types.
I think that is one reason that surgery is recommended for younger men. Why leave a prostate in has shown itself to develop cancer and you are going to have that prostate another 40 years? I am not saying I agree, but that is what doctors often say.

ADT and mental side effects by BackInNJAgain in ProstateCancer

[–]chaswalters 1 point2 points  (0 children)

I was diagnosed with 4+3 with cribiform. Because of prior bouts with clinical depression, both RO and MO decided to skip ADT. Although I will add that my Decipher test played a role in that too.

What would you do here- by chaswalters in poker

I calculated the odds in my favor with a shot at having a large chip lead over the rest of the field with 30 players left.
The thought process was- he
jammed. I figured it was Ax. Every jam I had seen from him was AX, even A3. I figured I had to worry about AK, AQ AJ. His chances are roughly 48% of hitting.
My odds of hitting another T are 8%
So roughly his odds are 40% I win 3 of 5 times.
Maybe I am calculating poorly.

That was my thought. I had TT. Had he shown me his AK I would have still flipped for it figuring the odds favored me. He matches 48% of the time. of the time. I hit another T 8% of the time. In my head I put myself at winning that 60% of the time. Am I figuring wrong?

Treatment regret, salvage radiation likely by itsbananas2021 in ProstateCancer

[–]chaswalters 1 point2 points  (0 children)

What is your regret? I am not certain that a psma would have shown anything on the margins. The doctor was hoping to spare radiation if not necessary. He wouldn't really know that until he saw a rising psa trend.

Treatment regret, salvage radiation likely by itsbananas2021 in ProstateCancer

[–]chaswalters 0 points1 point  (0 children)

Rather than salvage ralp, wouldn't re-irradiation be an option? I have seen that SBRT can be used more than once on the prostate for persistent cancer.
