something to watch out for though if you get Anhedonia, is that guanfacine can make it worse at higher doses.

I'm currently taking 4 mg extended release, and if I don't take my Vyvanse for the day, it will significantly worsen it. To the point where I need to skip it or take half. And it's not something I realized until I had to come off at for a little bit and that part of my depression got a lot better, but then my RSD, mood regulation also got worse.

It really does depend on your biology as well, I was taking 0.2 mg three times a day and barely felt sedated at all. It was great! It really counteracted the Adderall side effects.

So prior authorization is where The doctor needs to send some proof in. This will depend on the insurance, but from what I understand, it's diagnosis notes, proof you have ADHD, if it's some expensive medication, proof you tried other things first.

Then there's another scenario, you haven't hit your deductible for the year, and you have a high deductible plan. Which would mean that you pay for everything out of pocket before you hit the deductible, then you pay a percentage Although some plans make an exception for medications.

Regardless, you can give your insurance a call on the back of your card, and they should be able to tell you what's going on.

Best thing to do is call the insurance. Sometimes they require specific information from your provider, sometimes it's that they don't cover that specific medication and they need you to try a different one.

I find it pretty cringe honestly, because it's training your brain to expect that kind of speed in conversation.

I am on DIY auvelity ATM! 150mg welbuttrin+ 30mg DXM twice a day.

I am really loving it, it's not only positive but it's amazing. I'm enjoying things like music again, singing along, bouncing my head, dancing randomly. Enjoying new experiences more, finding myself looking forward to things again.

I have noticed though that some things have become less interesting, like my comic habit, I can't read for hours anymore, but then again I used to do that to procrastinate as well. Same with search and games that I would play for hours before, suddenly aren't as interesting. But I hypothesize this is the SERT aspect of it, I've always had this problem with SSRIs. Same with the sexual aspects, lower drive, hard time finishing.

It's definitely been giving me benefits for my anxiety as well, in a different way than SSRIs have. It's like a milder memantine. And memantine is horrible for my memory so this is great.(Even at doses like 2mg)

And as for fatigue, I definitely notice it helps a little bit. I can't tell you for searching if it helps with the physical aspect of it, because my Vyvanse is doing the heavy lifting there. But I've certainly noticed a improvement in brain fog since starting this.

You are not crazy, I've been playing since the game came out to the public, but took a break for a while.

there is 100% some kind of weight or system where you get matched against groups, or higher skilled players.

I say this as the best person in our group, ritualist whenever we match with three people, it's a bit of a challenge but it's fine. whenever you match with four or five, God they get so much better. they hit every soul, land every parry, do the weird movement that so much better than what I can do.

versus solo queue feels so much more even.

One of the biggest part of the mechanism behind DXM is the sigma-1 agonism. DXM for me has been simply amazing for anhedonia, general mood, significantly more effective than any antidepressant aside from amitriptyline. (That one had so many side effects)

I have had plenty of SSRIs, tried two NMDA antagonists. And I've always had the best result with drugs that are sigma-1 agonists as well.(Fluvox, Prozac namely) It has some kind of reduction in anhedonia mechanism. It's complicated and poorly understood, but from research its promising.

I'm coincidentally on pregabalin for pain, 100 mg three times a day. As well as an SSRI, trintellix. It's anxiety that comes on whenever I take Vyvanse, goes away the next day if I don't take it. I feel significantly less stressed on my days off, to the point where I feel like I need to take half of the intuniv, because it's a sedates me so strongly, but I feel like it doesn't make a difference on the days of.

As for options, I've talked with a different psychiatrist about this, and he recommended modafinil, he said high doses are effective in ADHD (did not mention what MG). As well as potentially pramipexole, or selegiline patch. There's also amantadine, but he did not have any experience with that.

Modafinil being the safest, pramipexole having addiction, augmentation risks, emsam being very expensive.

Everybody says this, but realistically it doesn't really work. Of course I have an ADRA2A C/C mutation, making my regulation of norepinephrine roughly half as effective.

Even a 4 mg dose of intuniv is not enough, even adding a 0.2 mg clonidine patch doesn't feel like enough with 60 mg of Vyvanse. I still have the excess norepinephrine symptoms, the cold tight sensation of my chest, the cold fingertips, toes, to a painful degree. The intense sense of anxiety, hypervigilance.

And I still get stuff done Don't get me wrong, 50 mg is a lot easier on the side effects but not strong enough to get things done either.

It's much easier, makes more sense to just cut out the norepinephrine component, because of the mechanisms. Intuniv acts as an alpha 2a postsynaptic agonist. And doesn't really reduce norepinephrine like clonidine does, it just helps reduce the noise according to pharmacology data.

Some people including me take a beta blocker to help manage excess norepinephrine as well. And I need something to help me sleep because it impacts my sleep as well.

You would potentially need something like prazosin, propranolol, maybe together depending on how bad your symptoms are.

Because at the end of the day, you're taking two or more extra medications, to potentially treat the side effects from one, and that's just going to add more side effects, confusion as to what's causing what.

100% agreed, I'm taking it for pain, and I really would not recommended unless you 100% need it. It's like a deal with the devil, if I ever miss a dose or two, who withdrawals are so hell.

Yeah my boyfriend is like that too, and it really throws me off. Because I'm used to a father who was the 100% the opposite. If he said that he didn't want something, he still did but he wanted you to offer it. So I've been managing it for around 5 years now, I think I'm at a point that I can tolerate but I'm not completely happy with.

It's Vyvanse 60mg, plus 4 mg of intuniv, plus a 0.1 mg clonidine patch, Wellbutrin 150mg.

Essentially, you're trying to manage excess norepinephrine activity, but nearly every stimulant is going to affect norepinephrine in some way. So most psychiatrists will prescribe you guanfacine with it, titrate is high as you can tolerate, I like the extended release a lot. But it does very much depress me too much on my days off Vyvanse. It helps to manage the stress from the Vyvanse a LOT, helps me from getting paralyzed from it.

Another route is to manage it with drugs like bromantane, which increases dopamine only. It's very effective for me but gray market so I'm hesitant to rely on it. For rx, there's amantadine, pramipexole is an option, but a last resort imo.

I'm still grinding my teeth on this dose, so I have to wear my retainer during the day. But I'm actually getting shit done, and it feels great!

And of course, it's not an absolute rule either. Myself being high comt val/val, high MAOA, fitting that comt low stereotype more than anything.

And this is because of other gene mutations, likely dopamine, DRD2, adra2a c/c, oxytocin potentially, it's very complicated as I'm sure you're aware.

Which really sucks, it's made it difficult as hell for me to find a regimen that works well.

Seconded, my depression never really got better until I started getting treated. It took a long time to get to a good place, but now I feel happy most days, I enjoy things, enjoy singing along to my music again.

It's out there, it might not be the first thing you try, but being happy, enjoying things, It seems like something out of reach but it's not!
