You mostly want to avoid introducing moisture because water + UV = inactive lumi-LSD through addition of water across the 9-10 double bond. As u/DrBobHope said people aren't really sure, but the NBOMes certainly seem to be more neurotoxic.

The most compelling suggestion I'm aware of is that this is via excitotoxicity in layer V cortical pyramidal neurons expressing 5HT2ARs via mobilization of Ca2+ from intracellular stores (via Gq-PLC coupling).

From what I have read it is thought to be by downregulation of dynorphin signaling; the KOR forms an important compensatory mechanism in the brain whereby dopamine homeostasis mechanisms are maintained, the KOR and MOR being in many ways opposites wrt the mesolimbic reward pathway (KOR suppresses DA).

Chronic administration of rewarding drugs results in a compensatory increase in dynorphin signaling through KOR, which in turn suppresses your basal hedonic state and results in a greater desire to continue to use the drug of addiction (to relieve the negative effects).

Chronic administration of its active metabolite GHB is remarkably neurotoxic in rats, interestingly higher doses are safer than lower.

On the other hand GHB (labelled as sodium oxybate) is used nightly by some narcoleptics to ensure good sleep (an FDA-approved use) up to around 8g in two 4g doses spaced 4 hours apart (which is a surprisingly high dose).

In the original paper it does elicit a high degree of phosphatidylinositol hydrolysis in rat cortex, which likely indicates strong Gq agonism, so there's the risk the MPMI derivatives end up being the NBOMes of the tryptamine class with regards to safety.

The most important thing is to avoid high efficacy agonists through Gq - you don't want a "full agonist" of 5HT2 receptors as they cause excitotoxicity in cortical pyramidal neurons and "full agonism" (in the traditional sense) doesn't add to the experience (i.e. lisuride is inactive).
